GRADUATING HIGH SCHOOL SENIORS
SCHOLARSHIP APPLICATION GUIDELINES
Applications must be received by the Foundation before 4:30 p.m., March 3, 2011.
Each scholarship has specific eligibility criteria. Please carefully review criteria, available on our
website, before submitting an application.
The application can be downloaded from our website in Word format for completion, but you must mail
a hardcopy to the Foundation.
The DEADLINE is not a postmark date. Applications must be RECEIVED by the Foundation by the
Deadline. You can deliver them in person, by mail, courier or package delivery service. To ease your
mind, you may wish to get a return receipt confirmation of delivery. If delivering in person, please visit
our website for directions. We do not accept applications via email or fax.
High School Seniors
Please submit the following documents, in the following order:
2011 Graduating High School Seniors Scholarship application form (complete a form for each scholarship –
be certain to enter the scholarship name on the top line.)
Statement of Activities
List of scholarships pending and/or awarded
Personal statement/essay - see individual Scholarship Guidelines for required topic
Official school transcript and documentation of recent SAT scores
Letter of recommendation from a guidance counselor, faculty member, and/or principal
Statement of Financial Need. Provide a brief description of how this scholarship will be helpful to you and
your family) (Submit only if required by the individual scholarship)
Other information as may be required specific to the scholarship for which you are applying
Please remember:
- to fill out the application(s) completely – a separate application is required for each scholarship
- to request your transcripts & letters of recommendation several weeks prior to the
Foundation’s application deadline. (March 3, 2011)
- if transcripts come directly from the school, they still must be received by the Foundation
by the scholarship receipt deadline (March 3, 2011)
- website generated or faxed transcripts are not official and are not acceptable
- do not apply for scholarships for which you are not eligible
- to be certain your name appears on every page
- late and/or incomplete applications will not be considered
- to mail the FAFSA Report Summary form to the colleges (you must have previously submitted
the Free Application for Federal Student Aid (FAFSA))
- multiple scholarship applications may be mailed/delivered in the same envelope
- do not submit your application:
o with any staples
o with CDs, DVDs or other electronic formatted materials
o with documents that are not on 8” X 11” paper & easily black/white photocopied
o with information on the reverse side of the pages
o with your full FAFSA report, Student Aid Report or other reports containing personal financial or
heath information.
