                                         Survey of Towson University undergraduates

This semester, the students in Sociology 391 (Research Methods) are conducting a survey concerning student life
and the Sociology program at Towson University. As a student enrolled in this Sociology class, you have been selected to
participate. The information we collect will be used by students in Sociology 391 and by the faculty to improve the
sociology program. Your responses will be kept strictly confidential and anonymous. Participation is voluntary. You may
skip any questions you feel uncomfortable answering, or discontinue the survey at any time. Your participation will be
greatly appreciated.

If you have completed the survey in another class this semester, please place the blank questionnaire in the box at
the front of the room. Completion and return of the questionnaire indicates your voluntary informed consent to
participate in this research.

If you have any questions or concerns, contact Dr. Marion Hughes, Assistant Professor of Sociology at 410-704-2930 or the
Chairperson of the Institutional Review Board (Dr. Patricia Alt, 704-2236).
