							                                     Using Forms Electronically

The forms in this appendix were imported into Microsoft Word as jpeg files. Therefore, you will not be
able to adjust or modify the structure or design of any of the included forms.

You can use the forms provided in this appendix in two different ways:

         You can print hard copies of the forms, or you can
         Create electronic copies of the forms and complete the forms using Microsoft Word text
          boxes.

If you choose to copy and complete these forms electronically you must create text boxes to fill in the
appropriate blank areas on the forms. Just draw a text box over the blank area you want to complete
and type the information in the text box.

You can save your new completed form to an appropriate file after you have filled in all the blank areas
on the form. Your completed electronic forms can be a printed hard copy, e-mailed and/or stored in
your computer.
