An "affidavit on information and belief" is an affidavit which is not based on the personal knowledge of the affiant, but rather on a good-faith belief by the affiant that the information is true. The source of the information and the grounds for the belief must be provided. An affidavit made on knowledge and belief is insufficient as an affidavit unless authorized by a special statute or where those matters sworn to are of necessity opinions or conclusions of law.
