JOB PURPOSE:

Under supervision of the Chief of Clinical Health & Prevention Services, the Immunization Program RN is responsible for
administering vaccines to at-risk individuals and for overseeing provision of immunization services in Harris County Public
Health & Environmental Services (HCPHES) clinics. Responsibilities also include the supervision of the immunization
program staff and annual updates to the Immunization Program Policies and Procedures. Immunization Program policy
development and changes will be in collaboration with the Chief, Disease Control & Medical Epidemiology and the
Manager of the Clinical Quality Management Program.
TASKS & RESPONSIBILITIES:                                                                                               %
Clinical Duties:                                                                                                        55
1. Review all existing immunization records (paper copies and electronic databases – Insight & ImmTrac).
2. Determine vaccine(s) client needs during present clinic visit.
3. Assess client for any contraindication(s) to vaccine(s).
4. Provide client/parent/guardian with vaccine specific Vaccine Information Statement (VIS).
5. Review paperwork for client/parent/guardian’s written consent for immunization(s).
6. Complete “Immunization Registration and Consent Form” with date and signature.
7. Review vaccines to be administered with the client/parent/guardians and obtain consent prior to giving the
     vaccine/immunization.
8. Administer vaccines in accordance with established HCPHES Immunization Program policies and procedures
     and provide client/parent/guardian with copy of the immunization record.
9. Complete real time Insight data entry to document client’s receipt of vaccines.
10. Give copy of vaccine administration record (Insight or C-102/C-104) to client/parent/guardian.
11. Review immunization paperwork for accuracy and completeness before giving it to Immunization Services
     Specialist.
12. Counsel client/parent/guardian about when to return for subsequent vaccine doses.
13. Carry out emergency procedures as needed for possible vaccine reactions according to HCPHES Emergency
     Medical Care Standing Delegation Orders.
14. Assist with monitoring and maintaining the vaccine and pharmacy inventories.
15. Follow approved medical record policies and procedures, including those dealing with patient confidentiality.

Administrative Duties:                                                                                                  40
   1. Develop, review and update policies and procedures for immunization-related activities.
   2. Supervise provision of immunization services by HCPHES LVNs and clerical staff as well as contract staff
      providing immunization related services.
   3. Participate in clinical quality activities such as immunization recall and reminder letters.
   4. Complete required Texas Department of State Health Services reports to include monthly immunization
      reminder recall activities reports.
   5. Develop and facilitate immunization training/in-services for HCPHES and contract staff.
   6. Conduct ongoing QA review to document consent for client’s participation in the immunization registry.
   7. Communicate with immediate supervisor and/or pre-identified HCPHES staff on any software issues related
      to INSIGHT.
   8. Participate in Random Moment Time Study (RMTS) by properly documenting activities as directed.

Other Duties                                                                                                            5
     1. Participate in meetings, trainings and/or exercises provided by HCPHES or other agencies as directed.
     2. Responsibilities include other duties as assigned, including special tasks involved in responding to an
          emergency event.

UPON ENTRY TO THIS POSITION, AN EMPLOYEE NEEDS THE FOLLOWING KNOWLEDGE, SKILLS, AND
ABILITIES IN ORDER TO PERFORM THE JOB FUNCTIONS IN A SATISFACTORY MANNER:
EDUCATIONAL REQUIREMENTS:
Bachelor of Science or Associate Degree in Nursing from an accredited college or university is required.


LICENSE(S) AND/OR CERTIFICATION(S) REQUIRED:
Current licensure as an RN in the State of Texas by the Texas Board of Nursing.
Current Health Care Provider CPR
Valid Texas driver’s license and reliable personal transportation.
EXPERIENCE REQUIREMENTS: (Amount and Type)
At least two years of supervisory and leadership experience acquired while working in a public health agency, hospital,
university or a similar agency is mandatory. A minimum of two years nursing experience in a public health, home health
or health service delivery setting is also required.
A minimum of 1 year of experience in screening immunization records of infants, children, adolescents and adults.

Experience requirements may have been met simultaneously.

COMPUTER PROFICIENCY: (Be specific as to the amount and type of software and/or hardware knowledge required and the proficiency level
of skills required, i.e., basic, intermediate, or advanced)
Basic proficiency in Microsoft Outlook, Word, Access and Excel.

SPECIAL SKILLS AND/OR ABILITIES:
Must maintain and display cultural sensitivity to community residents seeking immunization services.
Willing to work a flexible schedule including weekends and evenings as needed.
Bilingual (English/Spanish) is preferred but not required. .

PHYSICAL REQUIREMENTS: (if any)
Must be able to lift and carry at least 25 pounds.
May be required to work long and irregular hours during an actual or potential public health and/or infectious disease
emergency.

Guidelines governing practice are available through the Texas Nurse Practice Act, Pharmacy Act, Pharmacy D
Regulations, HCPHES Immunization Program Policies & Procedures and Immunization Program Standing Delegation
Orders. The employee is responsible for obtaining mandatory continuing education that is required for re-licensure in
Texas.
