I should be obliged if you could transfer the agency of my policy to Messer & Matthews Ltd
of 1 Riverview, Walnut Tree Close, Guildford, Surrey, GU1 4UX, whom I have appointed to
act as my financial advisers for the future.

Please also ensure that my new advisers are granted access to any information they require in
order for them to advise me fully regarding my policy, and that all copy correspondence
regarding the policy should be sent to them.

I understand that no alterations may be made to my policy without my written consent.
