     20xx Annual Report & Strategic Plan Outcomes Assessment
    Instructions: The Department Annual Report contains nine sections: Introduction, Strategic
Plan Outcomes Assessment, Liberal Arts/Student Persistence Activities, Staff Activities/Human
Resources, Programmatic/Operations, Continuous Improvement Planning, Additional Data,
Considerations for Subsequent Strategic Plans, and Remarks. This template provides questions
or comments to help complete each section.
    The Strategic Plan Outcomes Assessment section should include action plans that the
department identified in their strategic plan. Be sure to address how you assessed your progress
and what type of data was collected. An example is provided for you.
    The Staff Activities/Human Resources section should include information on your staff
activities during the year. Be sure not to include names or other individually identifying
information.
    The Additional Data section may include benchmark or trend data that your unit uses in its
strategic planning.

Action Plan: Use Image Now software to streamline office processes and to eliminate paper
file storage.
Progress Description: Image Now is being utilized to store cash reconciliation sheets for all
dining service locations reducing the volume of paper copies filed for audit purposes. All
invoices are being electronically scanned and office processes that are performed on these
invoices moved to the use of Image Now software features rather than being done manually.
Training for dining unit manager on the use of the software to access electronic files created
occurred in May and will enhance efficiencies of manager as well as eliminate another paper
copy and files. Full functionality of the software for accounts payable is now complete.
