Complex Continuing Care HOSPITALIST
(Part-time) Complex Care is changing! At St. Joseph's Health Centre Guelph, we are redeveloping our Complex Care Programs in our pursuit of excellence in service to our community. We are interested in finding an individual that shares our passion for excellence! You will be responsible for the medical management of patients in the hospital within our Complex Continuing Care program. You will be supported by a dedicated group of fellow physicians, nurse practitioners, nurses, therapists, administrators and support staff. As a member of the multidisciplinary team, you possess strong interpersonal and leadership skills combined with a demonstrated ability to facilitate change. You have excellent clinical skills, continually strive to improve patient outcomes, and value a sense of community. You are eligible for certification with the College of Family Physicians of Canada or the Royal College of Physicians and Surgeons of Canada. This is a salaried position with benefits if desired.
Position Description The hospitalist will be responsible for working with the interdisciplinary team in gradually growing the Functional Enhancement Unit (slow stream rehabilitation) to a 10 bed program. They will provide medical care for the 8 bed ALC transition program and the long stay complex care patients. There will be a nurse practitioner to assist the hospitalist(s) as well as a dedicated group of family physicians and an

interdisciplinary team. If there is an interest in palliative care, the hospitalist(s) may work with Dr. Will Ruddock in the development of our 15 bed palliative care program. Call as part of Core Group 1 in 8 weeks with stipend Monday to Friday, 20 hrs per week hours are flexible The hospitalist(s) will be responsible for working with the interdisciplinary team in gradually growing the Functional Enhancement Unit (slow stream rehabilitation) to a 10 bed program. They will provide medical care for the 8 bed ALC transition program and complex continuing care patients. Must be CCFP or Royal College eligible and have own CMPA coverage
