					This document should be filled out by an interviewer during and/or immediately following
an interview to evaluate the applicant’s strengths and weakness for a particular position.
This form provides standard language and questions that an interviewer should consider
when evaluating a job applicant, such as the applicant’s relevant educational and
professional experience, communication skills, and ability to work with current staff.
This form can be modified to best fit the needs of an employer as it seeks to assess
interviewer evaluations made about job applicants.
