Thank you for your interest in being a volunteer at the 2012 NAMI National Convention, scheduled for
June 27-30 at the Seattle Sheraton Hotel (1400 6th Avenue, Seattle, WA 98101). Please complete this
form in its entirety. All applications must be submitted by April 30th, 2012. You will receive notification
of whether or not we are able to place you as a volunteer by May 31st, 2012. ***Please note there will
be an important volunteer orientation on Tuesday, June 26 (4pm – 5pm), all volunteers are
encouraged to attend.

Volunteer Opportunities:

       Registration Desk People who volunteer at a registration desk assist pre-registered attendees
        obtain their name badges and registration materials. Volunteers at the registration desk should
        be able to handle some pressure and crowds with a smile.

       Hospitality and Information Desk Volunteers at this desk answer questions about the
        convention, and provide information about Seattle & surrounding area. People working at this
        desk should be well-versed in local restaurants, sight-seeing opportunities, shopping, and other
        local attractions. Volunteers at the Hospitality and Information Desk will be working with a
        NAMI staff member from the national office.

       Registration Packet Stuffing We set up several assembly lines to stuff about 2,200 bags with
        registration materials and information about the convention. People who volunteer to stuff
        packets should be able to stand and walk for up to three hours (breaks allowed!) and should
        wear comfortable shoes and clothing.
       Floor Guides Volunteers working as floor guides will help escort people from the hotel to the
        streets of Seattle to the convention center. Volunteers should be able to walk and stand for two
        hours at a time and give proper directions.
          Volunteers who work two shifts attend the convention at no charge.

Please indicate below the times and days you are available to volunteer at the NAMI convention. Please
sign up for at least two shifts choosing from the Registration Desk, the Hospitality and Information Desk
or packet stuffing. If you are flexible as to when you can work and would like us to schedule you during
our periods of greatest need, please check all of the shifts that you are available. We will not schedule
you for more than two shifts.

        Important Note on Parking: Unfortunately, NAMI cannot cover the costs of
        parking, which can be quite expensive in downtown Seattle, for volunteers at our
        convention. We highly recommend that you plan to use public transportation.
