BID FORM
1. Submit bids in compliance with Documents dated September 29, 2006. The Owner reserves the right
to reject incomplete bid forms. Bids must be received at the Architect’s Office no later than July 12,
2007 at 5:00 p.m. Bids may be hand-delivered, faxed or sent via e-mail. Bids received after the
stated time shall not be accepted.
2. This Bidding document is not part of the Contract Documents, unless specifically referenced in the
Owner/Contractor Agreement.

3. Base Bid: The Bidder proposes to perform all of the Work required by the Contract Documents for the

6. By submitting this Bid Form, the Bidder certifies that Bidder has visited the project site, is aware of
existing conditions which affect the work, and has reviewed the Contract Documents, including the

7. Bid Qualifications: Submit bid qualifications and reasons for qualifications with this Bid Form on
attached sheets. Include impact of bid qualifications on time, cost or quality. Bid qualifications may
include: Cash flow requirements, assumptions for access to the work, assumptions for staging the
work, assumptions for protecting existing and abutting work, proposed modifications to General and
Supplementary Conditions, proposed modifications to drawings and specifications.
8. Allowances: List any Allowances used for the Bid in addition to those contained in the Documents.
Note that material allowances listed below shall be contained within the Base Bid amount indicated
above.
