responses focus on the innovation that the individual has developed.

Quality: Describe how the innovation has increased the quality of an outcome for a course, program, activity, or
operation of the institution. Evidence of outcomes should be provided.




Efficiency/Cost Effectiveness: Describe how the innovation has increased efficiency or cost effectiveness.
Evidence of efficiency/cost effectiveness may be manifested via student ratings, pre- and post-comparisons of time
involved, cost reduction, changes in perceptions, or outside consultation verifications.




Replication: Describe how the innovation might be replicated on or off-campus.




Creativity: Describe how the innovation has been a significant departure from or improvement to a past practice.




Timeliness: The innovation selected should not be more than five years old, but it must have existed long enough
to have been tested so that it meets the criteria above.
