[www.ljmu.ac.uk/corporate/spr/index.htm] and current UMF Regulations www.ljmu.ac.uk/umf
before submitting this form. Students are also advised to seek advice from their personal tutor or
other appropriate academic staff. Independent advice on student related matters can be obtained
by contacting Liverpool Students’ Union Advice Centre at The Haigh Building, Maryland Street,

This form should be completed where a student requests the Module Assessment Board [MAB] to give
consideration to timely, severe, acute and unexpected circumstances which have significantly affected
their performance in a module assessment(s) and where the possibility of alternative mitigation is inapplicable
or inappropriate. It is the responsibility of the student to submit PMC applications within the required timescale
and to provide appropriate, independent and impartial supporting evidence with the application.

Submissions must normally be made only within the five working day period immediately following
completion of delivery of the module or final coursework deadline or where the circumstances relate
to a post-delivery assessment event, within five working days of that assessment.

LATE SUBMISSIONS

After this time and prior to the publication of results, any PMC submitted in relation to the module(s) will be
deemed to be LATE and must be submitted with a justification for the inability of the student to submit the
application within the established timeframe (UMF reg C4.7.2 (ii) refers) www.ljmu.ac.uk/umf.

In considering late PMCs, the School PMC Committee is seeking evidence of exceptional reasons for non-
disclosure, such as medical reasons (or similar) as corroborated by objective impartial sources. Students
must provide relevant documentation. The School PMC committee will then consider whether there is
sufficient justification for late submission (UMF Reg. C4.8 refers).

Students submitting a Late PMC application are required to complete Section 4 of this form and
provide justification for the inability to submit the application within the established timeframe (UMF
Reg. C4.8 refers). Justification for lateness [such as medical reasons or similar] should be
corroborated by objective, verifiable, impartial sources and be listed in section 4.

Please note that PMC Submissions will not be considered if submitted after the publication of results.

                                                                                       Last Amended 7 October 2010
                                                                                         Student Policy & Regulation
If the PMC is accepted, the MAB [advised by the School PMC committee], may either:

 Defer assessment to the next appropriate opportunity [see also note 1 below], but only where the module
has been failed. If the module has been passed, then the mark will stand.
  or
 Refer the case to the appropriate PAB (where there is agreement that the circumstances have severely
affected a student’s performance and where other mitigation has not dealt fully with the case).

CONSIDERATION OF PMC WILL NOT RESULT IN A CHANGE OF MODULE MARK

Please complete the section below and submit the form and all supporting evidence to The Chair
of the School PMC Committee, via The Student Support Zones. You are advised to keep a copy.
You should obtain a receipt and retain this as evidence of submission. Students submitting
forms by post are advised to use the Recorded Delivery service.
