      and/or intermediate service provider (ISP) participating in the

    ◦ Mail Form 2290 to the address shown under Where To File. If
      you did not pay the tax using Electronic Funds Withdraw
      (EFT) or the Electronic Federal Tax Payment System (EFTPS),
