Consent and information sharing guidance for practitioners when completing CAFs 1. When we ask a family for personal information, we will: Ensure the family knows why we need it. Protect it and make sure nobody has access to it who shouldn’t. Inform the family of your choices about giving us information. Tell the family if we need to share it with other organisations to ensure they get the service they require. Make sure we don’t keep it longer than necessary. 2. In return, we ask a family to: Give us accurate information. Tell us as soon as possible of any changes to personal information. Tell us as soon as possible if they notice mistakes in the information we have about them as this helps us keep our information reliable and up to date. 3. At any time, a family can ask us for further details on: Any agreements we have with other organisations for sharing information. When we might be required to pass on information without telling the family, for example, when a child is in need of protection, to prevent/detect crime etc. How they can challenge the accuracy or use of the information we have about a family. How to get a copy of the information we hold about you. 4. When completing a CAF, please ensure: That the family understand the CAF is only completed with their full consent and involvement. The family understand that the CAF focuses on their needs and what can be done to support these. That you contact the CAF registry to register a CAF and that you explain this to the family. That you give the family the option of their needs being discussed at a multi- agency meeting or a smaller team around the child meeting. When we ask a family for information, we will comply with the law, including the Data Protection Act 1998.
