Lead Professional Multi-Agency Meeting – suggested format for agenda
1. Ensure venue is welcoming and free from interruption. Think about the provision of
drinking water/tissues.
2. Welcome everyone and thank them for attending.
3. Housekeeping – share any information about the venue and collect the names of
everyone attending; give apologies, state how long the meeting is likely to last.
4. Agree purpose of meeting – e.g ‘What has led us here?’ What do we want to achieve?’
You may wish to highlight that there will be time to explore what is going well as well as
key areas for concern.
5. Ask what is going well for the child, inviting views from the child/young person and family
first
6. Ask everyone else present to say what they think is going well, moving in a circle from
the left of the family (if present)
7. Recognise areas of strength/resilience.
8. Update on actions from last meeting
9. Explore key areas of concern, again letting the child/young person/family speak first
10. Agree priority areas for actions and next steps – this is where ‘who will do what’ is agreed
and ‘by when’ is agreed.
11. Agree intended outcomes – This is a time for everyone to agree how they will all know
that things have changed, including the family’s views on desired outcomes.
12. Summarise key actions
13. Agree the date of the next meeting and who should attend.
