PROCEDURE

1. Submit all documents for COO Registration (1 week before COO Endorsement)
2. Purchase COO Form
3. Start COO Endorsement
4. Renew Letter of Indemnity & Statutory Declaration every 1 year

COO Registration (First Time)

Manufacturer:

    1.   Particulars of manufacturer for products exported.
    2.   Company's registration certificate, Form 24 & Form 49.
    3.   Business license/ manufacturer's license.
    4.   Letter of Indemnity (Must be print on company’s letter head)
         -With Duty Stamp RM10
    5.   Statutory Declaration (Must be print on company’s letter head)
         -Must be certified by Commissioners for Oaths
    6.   Product cost analysis.
    7.   Copy of My Card (Authorize person)
    8.   Picture / Catalogue of product.

Exporter/trader:

    1.   Particulars of exporters & products exported.
    2.   Company's registration certificate, Form 24 & Form 49.
    3.   Certification from manufacturer for its manufactured product.
    4.   Letter of Indemnity (Must be print on company’s letter head)
          -With Duty Stamp RM10
    5.   Statutory Declaration (Must be print on company’s letter head)
         -Must be certified by Commissioners for Oaths
    6.   Product cost analysis.
    7.   Copy of My Card (Director only)
    8.   Picture / Catalogue of product.

COO Endorsement

Documents required (before exporting):
  1. COO Form (must be sign by director and company chop )
  2. Packing List (must be sign by director)
  3. Sales Invoice. (must be sign by director)
  4. COO Endorsement from origin country (3rd party endorsement only)

Documents required (after exporting):
  1. COO Form (must be sign by director and company chop )
  2. Sales Invoice. (must be sign by director)
  3. Packing List (must be sign by director)
  4. Bill of Lading.
  5. Customs Declaration Form (K2).
  6. COO Endorsement from origin country (3rd party endorsement only)
