We now have to ask the Scottish Qualifications Authority to approve this request. As
part of the process, they need to know that [First-name] will make use of any
assessment arrangements they agree to.

If you are happy for us to go ahead with an application to the Scottish Qualifications
Authority, [First-name] and [First-name’s parents/carers (if under 16)] should sign in
the box below, then return the whole letter to me at school by [insert date]. A second
copy of the letter is enclosed for you to keep.

My teachers have discussed these arrangements with me. I agree to them
and intend to use them.
