The meeting was called to order at 7:30 PM by President Steve Dood followed by the Pledge of Allegiance and
introductions.

APPROVAL OF MINUTES
Alice Davis made a motion to approve the January minutes; seconded by Ted Bowlus. Corrections on Page 2,
Resolution #PER-02-01-2010: It should read that Betty Woods made a motion “to approve the fact-finding
report” and not “motion not to approve” the fact-finding report. After corrections noted motion to approve
minutes carried by voice vote.

WARRANTS
Alice Davis made a motion to accept the warrants as submitted; seconded by Betty Woods. Discussion as
follows:

Pg. 1 – T. Benarth - $39.59 – This amount is for an approved re-imbursement for equipment used by employee.

Pg. 8 – Comet Construction - $3,500 – This amount was used to paint the clinic area. Rooms have not been
painted since the health department first moved here in 1995.

Pg 11 – Merck @ Co. – $15,076.52 – This amount purchased 40 doses of various vaccines, two of which cost
$9,235.80 (6 doses of Zostavax) and $3,908.16 (3 Gardasil).

Pg. 15 – Verizon Wireless - $539.95 – This amount is cell phones which are all Verizon phones. No contract
changes.

PUBLIC INFORMATION
Ken Ault, Montgomery Twp. Trustee AND District Advisory Council chair, reminded everyone that the annual
DAC meeting will be held on March 11 at 6:00 PM at the health department.

PERSONNEL ACTION ITEMS
Resolution #PER-01-02-2010                     Motion to Appoint Amy Jones, Director of Nursing
Betty Woods made a motion to appoint Amy Jones to the position of Director of Nursing. Ms. Jones started
completing her duties on January 25, 2010. Her annual salary is set at $60,866.73 and she will serve a 6-month
probationary period. Motion seconded by Fleming Fallon and carried by voice vote.

EXECUTIVE SESSION
Alice Davis made a motion that the Board go into executive session for the purpose of discussing, reviewing and
considering contracts and other matters. Staff members Pam Butler and Bill Ault and labor relations consultant,
Tom Grabarczyk were asked to attend. Motion seconded by Betty Woods and carried by roll call vote as follows:

PERSONNEL ACTION ITEMS
Resolution #PER-02-02-2010                        Approve Agreement between WCHD & SEIU
Betty Woods made a motion to approve the Agreement between the Wood County Health Department and SEIU,
District 1199 for the Period July 1, 2009 through June 30, 2012. Motion seconded by Fleming Fallon and carried
by roll call vote as follows:

Resolution #PER-03-02-2010               Motion to Approve Help Me Grow Coordinator Position
Betty Woods made a motion to approve the creation of a Help Me Grow Coordinator Position for the Wood
County Health District. The position will be a full time Union position in pay range J with a probationary rate of
$23.20 and a rate of $24.16 after probation. Motion seconded by Alice Davis and carried by voice vote.

Resolution #PER-04-02-2010                Motion to Appoint Beth Instone Help Me Grow Coordinator
Betty Woods made a motion to appoint Beth Instone to the position of Help Me Grow Coordinator. Ms. Instone
will start her duties on February 15, 2010 at a probationary rate of $23.20. Motion seconded by Ted Bowlus and
carried by voice vote.

Resolution #PER-05-02-2010            Motion to Accept the Resignation of LuAnn Hogrefe
Betty Woods made a motion to accept the resignation of LuAnn Hogrefe as Clinic Supervisor effective as of
February 19, 2010. Motion seconded by Carolyn Rava and carried by voice vote.

FINANCE ACTION ITEMS
Resolution #FIN-01-02-2010               Approve Amendment to Appropriations
Ted Bowlus made a motion to increase the appropriations for Lead Fund Salary Expenses by $4,000 and Lead
Fund Medicare Expenses by $100.00. Motion seconded by Alice Davis. These changes are needed to pay out
retirement balances for our sanitarian who worked this program. Motion carried by voice vote.

ENVIRONMENTAL ACTION ITEMS
Resolution #ENV-001-02-2010               FSO/Retail Food Establishment Complaint Policy
Sandy Flick made a motion to adopt Resolution #ENV-001-02-2010, the Wood County Health District Food
Service/Retail Food Establishment Complaint Policy as required by OAC Section 3701-21-02.5. Brad stated that
the environmental division is currently in the middle of a food survey and it was suggested we adopt such a
policy. Motion seconded by Fleming Fallon and carried by voice vote.

NURSING ACTION ITEMS – None.

HEALTH COMMISSIONER’S ACTON ITEMS – None.

FINANCE
   Members were presented with budget to actual revenue and expense figures and fund activity through
   January for their review and approval as requested. Additional transfers and advances have been properly
   approved as per the Auditor’s request. Board members will be informed of date of Budget meeting some time
   in February.

NURSING

Amy reported the flu/H1N1 season is winding down; however, we do have some H1N1 vaccine left.

ENVIRONMENTAL

HEALTH COMMISSIONER’S REMARKS
  Child Fatality Review Board to meet March 22
  Volunteer Recognition Dinner, March 1, to recognize volunteers who assisted at the H1N1 clinics and staff
  at the health department who kept things running smoothly during the flu season. Pam reported because of
  the clinics, we made a lot of good partners and connections.

PRESIDENT’S REMARKS – None.

ADJOURNMENT
Sandy Flick commended Jerry Bingham on the good job he did on television educating the public on healthy
Super Bowl snacks.

Since there was no other business to discuss, Betty Woods made a motion to adjourn; seconded by Sandy Flick.
