   GOLD BADGE SOCIETY 14TH ANNUAL SCHOLARSHIP PROGRAM
                                           January 2012
             Please read carefully, incomplete or late applications will be disregarded.
All scholarships are available to graduating, qualifying children of active, disabled, retired or deceased
firefighters or paramedics of the Chicago Fire Department. All scholarships will be awarded at the CFD
Monthly Mass, May 20, 2012, 8:30 AM. at Holy Family Church, 1120 West Roosevelt Road. Winners will
be notified by May 1st 2012 and are expected to attend the service to receive their scholarship.

                  HIGH SCHOOL SCHOLARSHIPS AVAILABLE TO 2012 8TH GRADE GRADUATES
                                       PLEASE CHOOSE ONLY ONE
    Helen Lydon Miller Memorial Scholarship. This is an award of $750.00 a year for 4 years. Applicant
must provide a report card (copy) from the first semester of the 2011-2012 school year proving a B GPA
and a letter of recommendation from either a teacher or the school principal. Student must maintain and
prove a B GPA for subsequent years.
     FF/PM Christopher D. Wheatley High School Scholarship. This is an award of $500.00 a year for 4
years. Applicant must provide a report card (copy) from the first semester of the 2010-2011 school year
proving a C GPA and a letter of recommendation from either a teacher or the school principal. Student
must maintain and prove a C GPA for subsequent years. This will be awarded to a qualifying female
applicant.
___ FF. Francis Joseph Foley High School Scholarship. This is an award of $500.00 a year for 4 years.
Applicant must provide a report card (copy) from the first semester of the 2011-2012 school year proving
a C GPA and a letter of recommendation from either a teacher or the school principal. Student must
maintain and prove a C GPA for subsequent years. This will be awarded to a qualifying male applicant.

                    COLLEGE SCHOLARSHIPS AVAILABLE TO 2012 HIGH SCHOOL GRADUATES
                                      PLEASE CHOOSE ONLY ONE

    James J. McLaughlin College Scholarship. This is an award of $750.00 a year for 4 years. Applicant
must provide a transcript from the first semester of the 2011-2012 school year proving a cumulative B
GPA and a letter of recommendation from either a teacher or counselor. Student must maintain and
prove a B GPA for subsequent years. This will be awarded to a qualifying male applicant.
___ John and Barbara Hoffelt College Scholarship This is an award of $750.00 a year for 4 years.
Applicant must provide a transcript from the first semester of the 2011-2012 school year proving a
cumulative B GPA and a letter of recommendation from either a teacher or counselor. Student must
maintain and prove a B GPA for subsequent years. This will be awarded to a qualifying female applicant.

   FF/PM Christopher D. Wheatley College Scholarship. This is an award of $1000.00 a year for 4 years.
Applicant must provide a transcript from the first semester of the 2011-2012 school year proving a
cumulative B GPA, a letter of recommendation from either a teacher or counselor. Student must maintain
and prove a B GPA for subsequent years. Winner is to be determined by a lottery of the top ten finalists.
  For all of the above scholarships, a lottery will determine the winner from qualifying applications. All judgments are final .

All applications must be postmarked no later than March 31, 2012. Please note envelope, ATTN:
