 Scholarship award announcements will be emailed to all applicants no later than Thursday, January 26,
  2012. Notifications will be sent to the email address provided on the scholarship application; please make
  sure that your email address is current.

 A scholarship application, received by the deadline, that is incomplete or incorrectly filled out will
  result in the applicant being ineligible for award consideration.

Qualification Criteria
    New students must have been generally accepted into the MBA program prior to submitting a scholarship
     application.

    Students must, at the time of the scholarship award meeting and when scholarship funds are applied to the
     student’s account:
           Have a minimum GPA of 3.3
           Be in good academic standing with LFGSM (see 2011-2012 Student Handbook)
           Be in good financial standing with LFGSM (see Tuition Planning 2011-2013 booklet).
    Scholarships range from 10% of tuition per class to a maximum of 50% of tuition per class on amounts not
     covered by tuition reimbursement.
    Applicants may apply for up to, but no more than, two (2) units per term and up to six courses/units per
     academic year. Any global practicum course will be counted as a regular course and at regular tuition. If a
     scholarship recipient does not take the number of courses/units in the terms for which the award is made,
     the unearned award will be revoked and returned to that particular scholarship fund.
    Students with 100% tuition reimbursement for any course are ineligible to receive a scholarship award for
     that course.
    Since the majority of these scholarships are need-based, it is extremely important that applicants “tell the
     story,” including any extenuating circumstances, in the essay portion of their application. NOTE: All student
     information is strictly confidential.
    Students on track to graduate in the top 5% of their class (typically with a GPA of 3.97 or better) are
     encouraged to apply for the Hotchkiss Scholar Scholarship. (See page 4 for details.)




     Thank You Letter Requirement
     Names of students receiving a scholarship award are sent to the donor of that particular scholarship.
     Award recipients are required to send an appropriate “thank you” letter to the scholarship sponsor. This thank
     you letter must be on paper — not a fax or e-mail — to the sponsor with a copy to Nanci Saraceno,
     Development Associate, by Monday, February 13, 2012.
         Mailed letters - Letters must be received by Monday, February 13, 2012. Please mail your letter
          ahead of time to ensure you meet this requirement.
         Delivered letters - Letters can be dropped off at the Lake Forest or Schaumburg campuses during
          regular business hours on or before Monday, February 13, 2012.
    Scholarship awards will be forfeited if an appropriate thank you letter is not received by the deadline.

Available Scholarships
Lake Forest MBA scholarships are made possible through the generosity of the following individuals and organizations
and are specific to Lake Forest Graduate School of Management.


NEW AND RETURNING STUDENTS (NEED-BASED)
    General Fund Scholarship
    Many alumni and other friends of LFGSM designate their annual gifts in support of a pooled fund for current and
    incoming students called the “General Fund Scholarship.” All applicants are automatically considered for this
    scholarship.

    Aldo Cabri Endowed Scholarship
    Established in memory of Aldo Cabri, MGR ’71, the Cabri Scholarship provides resources to current students who
    run into emergency financial situations such as unemployment, a reduction of tuition reimbursement or other
    unexpected financial difficulties during the school year.

    Joseph A. Kaule Scholarship
    Established in 2009 by Robert J. and Mary Linda (LFGSM Class of 2004) Gariano, the Joseph A. Kaule
    Scholarship provides support to female students with an undergraduate nursing degree in need of financial
    assistance to complete their MBA studies. This scholarship honors Linda’s father, Joseph A. Kaule, who was a
    successful pharmacist and a great influence on Linda’s life and career in nursing. Qualified applicants must be a
    U.S. Citizen or Permanent Resident and a current or incoming student in good standing with the School. Female
    students with a nursing background are encouraged to apply.

    Desmond and Jeanne LaPlace Scholarship for Not-for-profit Managers
    Friends of LFGSM Dez and Jeanne LaPlace created this scholarship in 1998 with the intention of supporting those
    committed to the not-for-profit sector. The scholarship supports students whose primary employer is a not-for-
    profit organization. It is designed to provide not-for-profit leaders who desire to make a career in that sector with
    the management skills necessary to successfully lead their organizations through an increasingly complex and
    competitive not-for-profit environment. (Note: Applicants must submit a supplemental essay. See page 9 for
    details.)

    Trustmark Scholarship
    This well-respected insurance company, among the School’s longest standing corporate partners in scholarship
    support, established this award in 1997. Trustmark is committed to supporting individuals who are working to
    overcome social and financial disadvantages. Qualified applicants will be current or incoming students who are
    in good standing with the School and can demonstrate financial need.

    Daniel J. Yomine Endowed Scholarship
    Qualified applicants for the Yomine Scholarship must be current or incoming students who demonstrate financial
    need and are employed in a manufacturing position. The Yomine Scholarship, established in 1987, is named
    in memory of Daniel Yomine, an astute businessman and longtime friend of the School.

RETURNING STUDENTS ONLY (NEED-BASED)
    Maurice F. “Moose” Dunne Entrepreneurial Endowed Scholarship
    Established in 2006 in honor of Maurice “Moose” Dunne, LFGSM’s first president and long-time faculty member,
    this scholarship supports students who embody the entrepreneurial spirit exemplified by Moose. To qualify,
    candidates must have completed at least 4 units and agree to be recognized as a scholarship recipient.

    Recipients are encouraged, to the best of their ability, to make a future contribution to the Dunne Scholarship
    Fund. (Note: Applicants must submit a supplemental essay, resume and letter of recommendation. See page 9
    for details.)


    Richard and Joan Syer Scholarship
    This scholarship was established in 1997 in memory of Richard Syer, long-time LFGSM faculty member. Joan
    Syer Bailar, current LFGSM faculty and board member, continues to support this fund. Candidates must be in the
    second half of the MBA program (unit # 9 and above) and must have provided for more than 50% of their
    LFGSM educational expenses themselves.


RETURNING STUDENTS ONLY (ACADEMIC-BASED)
    Hotchkiss Scholar Scholarship
    This scholarship was established in 2004 by LFGSM Hotchkiss Scholars to foster academic excellence by
    supporting students who demonstrate outstanding achievement in the classroom. The Hotchkiss Scholar Program,
    started in 1984, honors students in the top 5% of their graduating class.

    The Program is named after Dr. Eugene Hotchkiss III, President Emeritus of Lake Forest College who served on
    the LFGSM Board of Directors for over 20 years. Applicants must be in the second half of the MBA program
    (unit # 9 and above), be on track to graduate in the top 5% of their class, and agree to be recognized as a
    scholarship recipient and help promote the Hotchkiss Scholar Program. Students with a 3.97 or better GPA are
    encouraged to apply. Demonstrated financial need is a secondary consideration in awarding this scholarship.


POST 9/11 VETERANS (NEW AND RETURNING STUDENTS)
    Yellow Ribbon Scholarship
    If a veteran is approved for Post 9/11 GI benefits, the Veterans Administration will pay up to $17,500 towards
     tuition for the 2011-12 academic year based on the level of approval. If a veteran is approved at the 100% level
     and tuition exceeds this amount, LFGSM and the VA will share the remaining tuition balance equally as a
     Yellow Ribbon Scholarship Award. Please go to www.gibill.va.gov for further information.

To apply for a scholarship, you must submit these three documents:

           1. 2011-2012 Lake Forest MBA Scholarship Application for Terms 3 & 4
           2. Complete the 2011-2012 FAFSA
           3. Signed copy of 2010 Federal Income Tax Return, or tax information can be directly imported from
              the IRS into the FAFSA.
           4. 2011-2012 Tuition Reimbursement Verification Letter

1. 2011-2012 Lake Forest MBA Scholarship Application for Terms 3 & 4: This application includes five sections
that must be completed in their entirety. Your name and date must appear at the top of each page. Incomplete
applications will not be considered. Please keep a copy of your application for your records.

You may use a typewriter to complete this hard copy of the application or download an electronic version at
www.LakeForestMBA.edu/scholarships. If you choose the electronic version, open the form on the Web site and then
“Save As” to your own computer. Be sure to name your file according to this format: first name last name
scholarship application (e.g., JohnSmithScholarshipApplication).

2. Signed copy of 2010 Federal Income Tax Return: This document must be submitted once per academic year; it
will be shredded after the 2011-2012 scholarship cycle is complete. Tax information can also be imported directly from
the IRS into your FAFSA. If you choose this option, your signed paper tax return is not required. Note: This option is
not available for applicants who are married and filed separately from their spouse. Although this information is used to
complete the FAFSA, filing the FAFSA does not satisfy this requirement.

3. 2011- 2012 FAFSA: This document must be completed once per academic year. First obtain a PIN at
www.pin.ed.gov, then complete and submit your 2011-2012 FAFSA electronically at www.fafsa.ed.gov. Your Student
Aid Report (FAFSA results) will automatically be sent directly to LFGSM via a secure file from the Department of
Education.

                 Academic-based Scholarship:
                    Hotchkiss Scholar Scholarship
                    GPA is the primary criteria for this scholarship. Students must be in the second half of the
                    program (unit #9 and above) and be “on track” to be in the top 5% of their graduating class.
                    Students with a 3.97 or better GPA are encouraged to apply.

                    A secondary consideration is financial need. Although this is an academic scholarship, the
                    EFC may be taken into consideration to distinguish among several top applicants whose
                    academic qualifications are equal.
                    Note: Hotchkiss Scholar Scholarship applicants with 100% tuition reimbursement for
                    any course are ineligible to receive a scholarship award for that course. See the
                    2011-2012 Lake Forest Graduate School of Management Scholarship Application

                    scholarship.

                 Veterans Scholarship:
                    Yellow Ribbon Scholarship

                 Sections A, B and C: Read carefully and fill out completely.

                 Section D: All applicants are considered for the General Fund Scholarship. Review the
                    other fund descriptions to see if there are others for which you feel you are qualified to apply.
                    You must indicate your scholarship choice with an “X” in order to be considered for an award
                    from any fund. Note: Because of very specific requirements for each fund, it is impossible to
                    qualify for all of them.

                 Section E: Essay Questions
                    Use the following Writing Skills Guideline in composing your essays:
                    1. Structure: The student understands the importance of filling out the scholarship
                        application according to the instructions in the guidelines. The applicant develops a
                        central theme and supporting data to clearly demonstrate financial need. Major
                        points are supported by details or examples. The conclusion summarizes the
                        student’s personal and financial circumstances.

                    2. Organization: The introduction leads into the applicant’s main message. The
                        applicant makes connections between the important points in the body of the
                        narrative. The conclusion summarizes the data clearly and concisely.

                    3. Readability: All sentences are complete, clear and correct. The applicant may use
                        subheads or bullet points where appropriate. The applicant has included all
                        pertinent information that will add value to his/her unique situation.

                    4. Style: A well crafted and interesting style is in keeping with the applicant’s
                        personality and the scholarship committee members’ needs and interests. The tone
                        is sincere and shows genuine need. Effective language is used throughout and all

                       concepts are clearly understood on the first reading. The applicant follows the
                       specific requirements stated in the application guidelines.

                    5. Mechanics: Major points are stated clearly. The applicant writes with clarity and
                       uses vocabulary correctly. Sentences are well constructed and maintain the flow of
                       thought. The application is complete and free from careless grammatical,
                       punctuation, and spelling errors.

                    6. Total word count for standard essay questions should not exceed 600 words.
