Aug. 2008 – Nov. 2010 The Arc of Prince George’s County, Largo, MD
Diverse position incorporating aspects of case management, fiscal management and
supervisory skills. Implemented a new statewide program. Monitored, supervised and
allocated state funding to five counties. Developed a program for students with
disabilities, facilitated the program and obtained funding for the program through
applying/writing a grant. Managed the agencies weekend respite house. Provided special
education advocacy to families. Taught varies workshops and trainings to the disability
community. Collaborated with community agencies. Responsible for the monitoring,
reporting and tracking of three grant funded programs, as well as, the respite
house budget. Trained and supervised college interns and volunteers.
Transitioning Youth Coordinator/ Eligibility and Access Coordinator
Oct. 2003 – Aug. 2008 Resource Connections of Prince George’s County, Lanham MD
Provided supports and necessary information to students and families on the transitioning
youth process and timelines. Attend IEP meetings as appropriate. Ensured that each
student completed the DDA application. Identified day and supported employment
providers for students. Collaborated with provider agencies to develop students Service
Funding Plan’s. Educated the disability community through trainings and transitioning
youth fairs on available community benefits/resources. Interviewed applicants for DDA
and provided eligibility determination recommendations. Attended Individual Planning
meetings and completed enrollment of individuals into the DDA Medicaid Waiver.
Community Health Outreach Worker II

Provided case management to developmentally disabled adults and children with the
primary focus of assisting clients in expressing their needs, wants and interests. Facilitate
and monitored the clients Individual Plan by conducting annual meetings, reviewing
progress, making modifications as necessary and developing goals. Reviewed service
provider records to ensure the plan was being carried out. Managed individual’s budgets
within the Individual and Family Services Grant. In home interviews and meetings to
ensure that the client was aware and effectively accessing community resources.
Family Service Coordinator/ Case Manager

Completed intake assessment interviews for clients with mental health diagnoses.

their children. Provided in-home services to assist families become self sufficient and

Foster Care Reunification Associate

Responsible for assisting biological parents meet requirements necessary to reunify
with their child/or children. Taught parenting skills classes and provide ongoing parent
training and support. Utilized community agencies, when appropriate and coordinated
services for foster children, biological parents, and/or foster parents. Worked as a team
member to determine appropriate placements for foster children.
Central Intake Coordinator

Interviewed clients requesting emergency assistance, determined what services the
agency and/or other community resources were appropriate. Coordinated services with
