                                           Evaluation Form
This form is designed to provide constructive feedback to the Pre-service Teacher and should be completed
with reference to the assessment procedures on p. 12. Please comment on both areas of strength and areas
requiring development.
Any Pre-service Teacher who receives “not met requirements” for any of the 6 areas of competence will be
graded as “has not met the overall requirements of the Professional Practice”.

A separate Overall Assessment Form also is to be completed based on this evaluation.

This form is to be completed by ticking “Met requirements” or “Not met requirements” for each of the
6 areas of competence.
