1.      About the Final Summative Assessment
1.1     Alignment

The purpose of this FSA Questionnaire Guide is to provide a battery of Assessment Questionnaires to
be used as Final Summative Assessment for each Learning Unit in the (Insert Qualification Title,
level) 4 Qualification.

                   Tip:
                   The FSA (Final Summative Assessment) takes place after each Learning Unit based
                   on the unit standards of the respective Learning Units, as well as at the end of the
                   qualification, based on the exit level outcomes and related assessment criteria
                   stated in the qualification description.

This FSA Questionnaire Guide must be read in conjunction with the Services SETA (Insert
Qualification Title, level) programme material – Learning Unit 1 which is based on the following unit
standards:

1.2    Assessment Conditions

                 Final Summative Assessment Rules:
                 The following final summative assessment rules are prescribed by Services SETA,
                 and must be adhered to in the assessment process.



1. The Final Summative Assessment is compulsory for all learners who want to qualify for
   certification.
2. Only learners who have submitted their portfolios of evidence (PoE) to the provider before the
   FSA date will qualify to do it.
3. The assessment is based on applied and integrated knowledge of all the unit standards relevant
   to the Learning Unit, and finally the whole qualification / learnership.
4. Learners need to prepare for the assessment and review everything they have learned and
   experienced during their learnership.
5. Duration of the assessment will be pre-determined and explained to all involved.

                 Tip
                 6. Learners need to write the names of their learnership qualifications, ID
                     numbers and their own names on the cover sheets of their
                     assessment/answer books/sheets.
                 7. Learners need to write the names of their learnership qualifications, ID
                     numbers and their own names in an attendance register that is kept with the
                     invigilator.
                 8. Invigilators need to check learner attendance records against their ID books
                     and ID numbers.
                 9. Important: make sure that any loose answering sheets or additional
                     answering books are clearly marked and stapled together.


10. The assessment will be conducted by registered constituent assessors.
11. Where specified, please number questions carefully.
12. Assessment can me conducted using different methods – this should be clearly explained to the
    learners during their preparation for them to know what to expect during the FSA.
13. The assessment can also take the format of a written assessment. With this method of
    assessment, a variety of questions can be asked. These may include multiple choice questions,
    short questions, short essay type questions and questions based on a case study.
14. Answers may be written in blue or black ink only. Answers in pencil will not be considered.

                 Tip:
                 15. Please make sure of the commencement time of the assessment. Late arrival
                     might lead to learner not being able to complete the assessment. No
                     additional time will be granted due to late arrival. Learners who arrive more
                     than 30 minutes late will not be allowed to do the assessment.
                 16. Learners need to be seated in the assessment venue at least 15 minutes
                     before the start of the assessment.

17. No materials shall be brought into the room or used at the assessment venue except those
    authorised by the invigilator. Bags, beverages, food, jackets and books need to be placed in
    areas designated by the invigilator and are not to be taken to the learners’ desk.
18. No electronic or communication devices will be allowed in the assessment venue, including cell
    phones, laptops, pagers, etc. Calculators are not allowed unless specified by the instructor and
    indicated on the assessment paper. Only non-programmable models authorised by the
    instructor will be allowed. It is the learner’s responsibility to ascertain whether the use of
    calculators is permitted, and, if it is, whether any restrictions are imposed on the types of
    calculators that may be brought to the assessment.
19. Learners may only have writing utensils and ID documents at their desks.
20. The invigilator has authority to assign seats to learners.
21. Learners may not borrow or receive assistance from other learners during the course of the
    assessment.
22. Learners are not allowed to communicate with one another in any manner whatsoever during
    the assessment.
23. The assessment will be conducted in English. The answers also need to be in English.
24. The use of a bilingual dictionary is allowed, however, the invigilator will check it for notes.
25. Learners need to provide their own bilingual dictionaries.

                  Tip:
                  26. It should be pre-determined if the assessment will be conducted as an open
                      book assessment. Any study notes in the learner’s possession should be
                      handed to the invigilator before starting the assessment. Learners with study
                      notes in their possession while writing the assessment will have their papers
                      disqualified. Any incident will be investigated thoroughly and the learner will
                      face serious consequences.

27. Unless in distress, learners are not allowed to visit the washroom during an assessment session.
    Such exits will be accompanied by an invigilator. If you need to go to the bathroom during the
    assessment, please hand in your assessment book. Only one person may go to the bathroom at a
    time.

                   Tip:
                   29. Assessments will be moderated by learning area specialists.
                   30. 20% of completed assessments will be moderated for consistency, fairness
                       and reliability.


31. Assessment and assessment scripts will be assessed by registered assessors and will form part of
    the final results of the learners PoE.
32. Learners with special needs need to inform the Certification or Outsourced Partner of SSETQA
    through their training providers and supply affidavits in case the assessment cannot be written,
    in advance. Learners experience any crisis situation and cannot attend the assessment, need to
    inform the Certification or Outsourced Partner of SSETQA through their training providers and

33. Learners are expected to conduct their assessment in an honest and straightforward manner.
    Where there are reasonable grounds for believing a violation of assessment protocol has
    occurred, the learner will be subject to the disciplinary procedures and sanctions.
