From a management perspective, hospitals are complex organizations, they are more
complex than most other types of business organizations.

A hospital is generally responsible to numerous sources of authority. This includes the
administrative arm, the medical arm and the trustees. It is a tripartite structure : Board of
Trustees- Medical Director-CEO.

From the outside, hospitals appear as cohesive organizations with a united sense of purpose
and a clear goal of providing high quality patient care. From the inside, however, hospitals
are comprised of several different components, each of which has distinct goals and roles.
Each of this components has different responsibilities. In reality, the boundaries are not
always clear about the areas of authorities.

The governing board is ultimately responsible to all what is going on in the hospital, both
administratively and clinically.

Although the medical staff is legally subordinate to the governing board's authority,
physicians are partially autonomous from the board through the structures set forth in the
medical staff bylaws.

There is a unique relationship between hospitals and physicians, and this relationship is not
without strain and stresses. This is one of the challenges in hospital management.

The management's role is to adopt the board's policies and to manage the hospital's
activities on a day to day basis.

Teamwork and and good communication practices are necessary to avoid conflict.

Hospital administrators have developed significant expertise in recent years in dealing with
the increasingly complex operational, market, and regulatory issues confronting hospitals.
But since the internal triad is not always agreeing on the directions and priorities, hospitals
often experience internal tensions and find it difficult to respond in a systematic way to
environmental conditions and changing community needs.

Given the challenges facing hospitals today it is imperative that the three arms of leadership
will work harmoniously as a team.
