Lease and Extension Agreement made on the (date), between (Name of Lessor) of
(street address, city, state, zip code), referred to herein as Lessor, and (Name of
Lessee), of (street address, city, state, zip code), referred to herein as Lessee.
Whereas, pursuant to the Lease (the Lease) dated (date), hereinafter called the Lease,
Lessee leased from Lessor the building located at (street address, city, state, zip code),
hereinafter called the Premises, and more specifically described in the Lease for a term
which expires on (date), a copy of which is attached hereto as Exhibit A; and
