ELIGIBILITY
EVIDENCE OF PROJECT ELIGIBILITY (Check the appropriate item if the proposed project meets the eligibility requirement.)
___ Project/program meets Federal eligibility requirements.
___ At least one sponsor is a public agency.
___ A letter or letters of sponsorship/commitment addressing all required components (local funds, maintenance, project priority,
status report, intergovernmental agreement) is/are included.
___ Project goes beyond normal, routine or customary elements of transportation projects and does not include typical maintenance
activities.
___ Local match meets required amount of the total project cost (17.21% for CMAQ and STP – Metro; or 20% for TE).

___ Project funding will not be used solely for construction of curb and gutter. (Any use of TE funds for curb and gutter must be
reviewed and approved by the FHWA.)
___ Project meets ADA requirements.

project? Include the agency responsible for the continued maintenance of the project and the anticipated maintenance and staffing
needs over the economic or useful life of the project.)

mitigation pool, planning studies pool, and TE funding programs. Only complete the appropriate section for the funds you are
requesting.
CONGESTION MANAGEMENT AND AIR QUALITY

10 points – Project in a local plan but not in the 2035 RTP (provide website link or appropriate page(s) from the local plan showing

0 to 10 points – Project in the 2035 RTP receives up to 10 points based on its 2035 RTP criteria score. This is a normalized score
(PPACG staff will develop):
Carbon Monoxide Emissions Reduction. Will the project or program reduce carbon monoxide emissions? How was the reduction
determined? Application should provide a detailed explanation of assumptions, facts, and methodologies; application should identify
who developed assumptions and facts, when, and what geography they apply to.

Congestion Reduction. Will the project or program reduce congestion? How was the reduction determined? Application should
provide a detailed explanation of assumptions, facts, and methodologies; application should identify who developed assumptions and
facts, when, and what geography they apply to.
Cost Effectiveness. What is the relative cost effectiveness of the project or program?
Congestion Management Process. Priority is given to transportation strategies and solutions from the congestion management process
focusing on the listed congestion management corridors in the approved 2035 Regional Transportation Plan.
Peak Hour Benefits. Is the project or program designed to emphasize congestion reduction during peak hours?
Public-Private Partnerships. Have financial contributions been committed to the project by private organizations/partners?
Benefits to Other Criteria Pollutants. Does the project or program reduce levels of other criteria pollutants (ozone, nitrogen oxides,
particulate matter, sulfur dioxide, lead)?

Secondary Benefits. Does the project or program have secondary benefits (for example: context sensitive, reduces other air pollutants,
increases public involvement in local and/or regional planning activities, improves emergency vehicle response time, improves access
to jobs, access for the disabled, etc.)?
Local Match. What percentage of the total project or program cost is local match?
Benefit Life. How many years of full emissions or congestion benefit (20 years maximum) will be received from this project or
program?
Social Equity. Does the project serve a low income or minority community? (“Serve” is defined as the project starting, ending, or
passing through a low income or minority community.)

Project Readiness. Is the project ready to be implemented (i.e., facility acquired, preliminary engineering complete, NEPA work
complete?)
Social Equity. Does the project serve a low-income or minority community? (“Serve” is defined as the project starting, ending, or
passing through a low-income or minority community.)

Multimodal Use. Does the project accommodate multimodal use (bicycle, pedestrian, equestrian, transit access, etc.) and provide an
alternative to driving?
Context Sensitivity. What is the relationship between the project and existing amenities, which include, but are not limited to, natural
areas, natural bodies of water, cultural sites, historic sites, parks, etc?
Project Cost and Estimated Benefit. What is the regional benefit as determined by number of users, context enhancement or other
benefits?
OFF-SYSTEM BRIDGE REHABILITATION / REPLACEMENT
Inspection Condition. Utilizing the lowest National Bridge Inspection Standards (NBIS) rating for Items 58, 59, 60 or 62, from the
latest inspection report (or justify the reason for using a previous inspection rating) for the bridge.
Remaining Life. What is the estimated remaining service life, and justification for this opinion, of the bridge? The evaluators should
review the written explanation as the basis of the scores.

MAPS, PLANS AND PHOTOGRAPHS (Please provide an 8 ½ x11map, color or black and white, of the project area showing as
many details of the project as possible. The map must have a north arrow, scale, and title of the project. It should clearly show the
project location, property lines, public facilities, state roads, and any other relevant information. Map must clearly identify the
proposed project site with beginning and ending points. Use of freeware internet mapping with aerial photos is recommended.)
