                    GROUP LIFE COVER FOR M, P, AND S BAND EMPLOYEES

The Group Life Insurance Policy can’t be ceded and therefore it is necessary to amend and
withdraw the cession and nomination forms and replace them with the attached form.

Furthermore, the Group Life Assurance Policy does not cover disability claims. The Stated
Benefit policy as indicated in paragraph 4.7 as per the Employment Conditions for Managerial
levels only covers accidental disabilities. The employees are also paying fringe benefit tax on
the premium of R 56.33 per month.

Therefore employees who so wish to nominate the proceeds of the policy to the Eskom
Financial Company or any external financial institution have to complete the cession
agreement form that can be obtained from the applicable financial institution. The employee
must submit the original cession form to the relevant financial institution and a copy thereof to
HR in order to file it on the personal file;

                                                       AND/OR

The employees can nominate their beneficiaries of choice by completing the relevant section
of the nomination form.

All M, P, and S band employees between the ages of 18 and 65

Group Life Policy Number 91471 for M, P and S band employees.

Commencement Date
1 January 2002

Benefit Sum Assured
Death Benefit equals three times (x3) salary plus such amount as shall be necessary for tax
liability.

Benefits Payable
In the event of Eskom receiving evidence of the death certificate and claim form of an
employee.

Exclusions
The liability of Eskom to pay the death benefit shall not arise if death, directly or indirectly,
arises from or is traceable to war or armed international conflict, (whether war be declared or
not.)

Terrorist or insurgency activities, uprising, civil commotion, rebellion, sedition, sabotage or any
activity associated with the aforegoing or the defence, quelling, investigation or containment
thereof by any security force, involving nuclear arms, biochemical agents or any other form of
war.
