                  5k Run/Walk and 1-Mile Walk on September 8, 2012
This year, I’m making a big impact on the fight to end prostate cancer by participating in the Great
Prostate Cancer Challenge on Saturday, September 8, 2012. (Insert personal story of how you have
been affected)

This year in conjunction with The Urology Group in Cincinnati, the Great Prostate Cancer Challenge
will take place in 36 cities across the country. Race planners expect to have close to 16,000 people
running in these various races to raise awareness and funds for prostate cancer. At the Cincinnati
race our goal is 1,000 runners.

Here’s how you can help:
   1. Participate in the Race. To register visit: www.TheUrologyGroup.net and click the “Register
Here” link. It’s $30 in advance/$35 on race day.
   2. Donate now. You can help me raise funds by donating now. Visit: www.TheUrologyGroup.net,
click on the “Register Here” link and then scroll to the “Donate Now” section.
   3. Sponsor the Race. If you or your employer would like to sponsor the race, please email:
       Prostate5KCincy@TheUrologyGroup.net.

All funds raised by the Great Prostate Cancer Challenge will go to ZERO — The Project to End
Prostate Cancer and their mission of eliminating prostate cancer deaths with research, advocacy and
awareness. ZERO funds the only nation-wide mobile prostate cancer-testing vehicle. ZERO has
helped screen over 100,000 men since the program’s inception in 2002. Money raised from the Great
Prostate Cancer Challenge will go towards screenings in the Cincinnati area.

Why is this so important to me, to you and to so many other people? Because prostate
cancer affects 1 in 6 men and will kill almost 30,000 men this year alone— that could be a family
member, friend, neighbor or colleague. Please help me to raise money to fight this disease. Nearly,
100% of men diagnosed with prostate cancer while still in the early stages are still alive 5 years from
diagnosis.

Thank you for your participation and your generosity. Together we can end prostate cancer.

Sincerely,

P.S. – Ask your employer if they will match your donation through their gift program!
