Payment and Cancellation Policy
Payment is due in full before the start date of the course. Class size is limited and is based on a “first
come” basis with regards to the date the course payment was received. Payments may be made via
check or credit cards (Visa and Master Card only). Checks and credit card payments should be made
to American Radiation Services, Inc. Credit cards will be charged the full amount upon receipt of the
credit card billing information. Checks shall be cashed upon receipt. If you are unable to attend a
course, you have the option to send a replacement or to transfer to a future course held within one year.

Credit card payments may be made via the telephone with the ARS Accounting Department or by
completing the Credit Card Charge information on the NORM Course Registration Form. The ARS
Accounting Department can be contacted at 1-800-401-4277.

For alternate payment options, or for companies participating in the Louisiana Incumbent Worker
Training Program, contact the ARS Accounting Department at 1-800-401-4277.

Course Cancellation
If a course is cancelled due to insufficient enrollment, you will be notified by phone or email at least
three weeks prior to the start of the course.

Course Locations
Baton Rouge courses will be held at the Hilton Garden Inn. The Hilton Garden Inn is less than a half
of a mile from the Baton Rouge airport.

needed to be enrolled in a NORM Radiation Safety Officer course three weeks prior to the course date
to prevent course cancellation. Registered students will be contacted three weeks prior to the
scheduled course date in the event of cancellation.

Cost of the course is $1,375 per student. Payment is due in full before the start date of the course. See
the “Payment and Cancellation Policy” above.

Class time is 8:00am to approximately 5:00pm. A one hour lunch break will be taken. Lunch will be
on your own. To register a student for a course, fill-out the attached “Student Registration Form” and
fax or email the form to 225-381-2996 or training@amrad.com. Class size is limited and will be filled
on a first come – first served basis based on the date the course payment was received.
