1 "SPORTS FOODS" in this survey INCLUDE:
a) Carbohydrate and protein rich products, like: bars, carbohydrate-energy gels, drinks
and powders; protein shakes, drinks and concentrates in powdered or liquid forms, etc.;
b) Sports drinks (carbohydrate-electrolyte drinks).
2 "SPORTS FOODS" in this survey EXCLUDE:
a) Supplements of vitamins, minerals, creatine, caffeine, carnitine, HMB and other legal substances;
b) Drugs or doping substances banned by WADA (World Anti-Doping Agency);
c) Herbal supplements.
3 "PRODUCT(S)" in this survey refer to "sports foods".

Survey title: Social Impact Assessment of Sports Foods Regulation
1 Purpose of the survey
Food Regulation in the UK and Europe is evolving, and this can impact on many aspects of sports foods,
drinks and supplements. The aim of this survey is to learn more about your opinions and perceptions
of sports nutrition products, how important regulation is to you, and what you expect from future
changes in regulation, as a potential user of sports nutrition products.
We hope that the information obtained from this survey will contribute to inform the UK and European
regulatory decisions on the future of these products.
2 Your participation
You have been asked to take part in this survey because you play sports or take part in exercise
on a regular basis. You don't necessarily need to be a consumer of sports foods.
You must be over 18 years old and live in the UK.
You must not be an employee of GlaxoSmithKline Nutritional Healthcare.
If you don't meet all the criteria above, please accept our apologies and decline to participate in this
survey.
This survey is being sent to more than 500 respondents, including sportspeople and health
professionals. We expect to receive back at least 60% of the questionnaires sent.
By accepting to take part in this survey, you will be only required to respond to a standardised
questionnaire, which contains 23 questions, and will take approximately 25 min of your time to
complete.
Your participation is fully computer-based, managed via internet. However, you can choose to
answer the questionnaire on a printed hard copy and submit it by post. If you choose for the latter
option, please e-mail me with your request and address, so I can provide you with a pre-paid envelope:

3 Benefits of taking part
There is no direct benefit to participating in this survey.
We hope that you will find it interesting to take part and that you gain further awareness of food
regulation.

Your taking part in this survey will be kept confidential.
All personal data and responses you disclose will be held in the strictest confidence, and only the
main investigators will have access to it. Your responses will be given a code number, will be
anonymously pooled with all other respondents' questionnaires, and cannot be traced back to you.
Your responses will be used solely for this survey and publications arising from this survey and
will not be available to any third parties.
Your data will be stored in a password-protected personal computer and locked filing cabinet, and
will be kept for a period of 5 years after the completion of this survey, when it will be destroyed.
Electronic communications (e-mails and exchanged data) between you and the investigators
will be treated under the rules of the Privacy and Electronic Communications (EC Directive)
Regulations 2003.
