							          HOW TO OBTAIN A LONG FORM BIRTH CERTIFICATE
 (THIS A BIRTH CERTIFICATE THAT REFLECTS BOTH PARENTS NAMES)

Someone who comes to the office wanting to apply for a passport must now submit an
             application that reflects BOTH PARENTS NAMES.
                                  Effective April 1, 2011

                           The applicant must do the following:

 1. Obtain an application from the division of vital records through this website:

 _by_mail/556813

 2. Applicant will complete questions #1-2-3. Questions #4-#5 do not have to be
 completed.
 3. Applicant will then submit to the vital records Erie Branch 1910 West 26th street
 Erie, PA 16508-1148. There is NO FEE to replace the current birth certificate.
 4. Applicant will submit the current birth certificate that they already have. Please
 note that the current birth certificate MUST be in GOOD condition to be able to
 submit it for a replacement birth certificate.
 5. Applicant will also submit a copy of their current valid driver’s license. (A
 passport is not a valid form of ID; due to no address appearing on it.)
      Please note that requesting a birth certificate through the central office which
       is in New Castle, PA will significantly delay receiving your new birth
       certificate and may take up to 8 to 10 weeks.
           Anyone who applied for a birth certificate in the year of 1972 and later
            should have received a birth certificate listing parent’s information.
           Any parent that did not list one parent at the time of the child’s birth will
            receive a birth certificate that shows “INFORMATION NOT
            RECORDED”
           Although you request a new birth certificate the information remains the
            same as listed in original documentation. A parent cannot add or delete a
            parent unless PATERNITY HAS BEEN PROVEN!
