All UNI/CARE System Users:

UNI/CARE Systems, Inc. is pleased to offer UNI/CARE Training Sessions immediately
following the 18th Annual Users’ Group Conference.

The Training Sessions will be held April 6-7, 2006 in Sarasota, Florida at the Hyatt Sarasota, the
same location of the 18th Annual Users’ Group Conference. Training Sessions will include
training for Pro-Filer™: Clinical Default Plans & Templates, Advanced HTML Functionality,
Your Billing Test Plan, Managed Care Authorization Review, Aviator Navigation Principles,
Efficient Scheduling, Report Writer, State Reporting Report Builder, and System Administration.
Each course requires the attendees to have received prior training of the offered subject or a
generalized knowledge of Pro-Filer™.

The registration fee for each training session (4 hours) is $99.00. There will be a $25.00 per
attendee late fee for all registrations received after March 16, 2006.

Please review the enclosed training literature, schedule and training descriptions and submit your
registration today to attend the UNI/CARE Training Sessions.

Please make check payable to UNI/CARE Systems, Inc. Payments must be postmarked before March 16, 2006 or late fee applies. You
may fax your training registration to Jamie Smith at (941) 954-2033, however your registration will not be confirmed until payment is
received. Please use your registration form as your invoice.


CANCELLATION/TRANSFERS:
If you need to cancel your training registration, a cancellation fee of $50 will be deducted from your refund. To receive a refund, your
cancellation must be received by 5:00 p.m. EDT, March 24, 2006. There will be no refunds after this date and no refunds for no-shows.
If you are pre-registered and unable to attend, you may authorize someone else to attend in your place. For your protection, the
substitute must present a signed statement from you at the on-site registration booth at check in.

CANONS OF CONDUCT:
The same Canons of Conduct for the UNI/CARE Users’ Group Conference apply for the UNI/CARE Training Sessions and they are as
follows:

Canons of Conduct are UNI/CARE Users' Group policies that have been implemented to allow for the revocation of conference
registration of those attendees who have been obviously disruptive and/or have attempted to sabotage the conference. All attendees
who attend are required to read and abide by the Canons of Conduct. Attendees at the UNI/CARE Users' Group conference must
properly register and display appropriate credentials. Attendees at the UNI/CARE Users' Group conference must conduct themselves
and their activities in a professional manner marked by integrity and a spirit of fair play. The Conference Planning Committee reserves
the right to determine if any activity is in violation of the canons of conduct and reserves the right to dismiss an attendee from the
conference with no refund. UNI/CARE Users' Group presentations, publications, programs or any other product will not contain technical
data or information that is proprietary, classified, confidential under U.S. Government/State laws, or controlled by non-disclosure
agreements.
