All Adult & Senior Fees include applicable OCA fees, locker + taxes
Senior -60 years of age as of October 1, 2012
* Adult (under 29) -28 years of age as of October 1, 2012
-Copy of proof of age must accompany membership application
** Friday Night Mixed fees are for this league only/no sparing or curling in other leagues
*** Fee charged to returning members who have not submitted membership form & fees by October 5, 2012
SHARES: Shares may be included with your fee payment or deferred as follows: Every adult/senior member must
own 1 share AFTER their first year of membership. This is a ONE-TIME expense. All shareholders are entitled to
vote at meetings of the corporation. ONLY persons with 1 fully paid share are eligible to stand for the Board of
Directors. Companies or organizations owning shares may nominate one employee or official for club membership for
each share it owns, subject to approval of the Board of Directors and payment of membership dues.
Dates to Know:
Apr. 9-13/12 - First paid/first served with priority given to 2011/12 returning league members
Apr. 24/12 - BCC General Meeting 8:00 pm
May 1-Aug. 31/12 - Office will not be open on a regular basis (email & voice mail accessible)
Oct. 2/12 - BCC Shareholders Meeting 8:00 pm
Oct. 3/12 - Open House/New Members (applications received from 3:00-9:00 pm)
Oct. 9-10/12 - Instructional Clinics from 6:30-8:30 pm
Oct. 15/12 - League curling starts
Assumption of risk/release liability agreement.
I understand and agree that there is a risk of injury when participating in any physical activity including curling. I
release The Barrie Curling Club Limited from all liability arising from personal injury or death and/or damage or theft
of my personal property.

Refund/Credit Policy:
Refund or Credit will be issued by the following dates as described:
Oct. 12/12 -A full refund will be given less the $30.00 administration fee.
Nov. 30/12 -No refund. A medical certificate is required to receive a pro-rated credit note. The credit
note must be used within one year of issue. A $30.00 administration fee applies.
Jan. 4/13 -No refund or credit
