INTRODUCTION:

Thank you for choosing First Capital Mortgage, LLC for your real estate loan. We are pleased to assist you in
securing home financing. As used in this disclosure, the words “we”, “ours”, and FCM means First Capital
Mortgage, LLC. The words “you, “yours”, “applicants”, and “borrower” mean any and all of the applicants
applying for this mortgage loan.

The purpose of this Initial Application Disclosure is to provide you with:
        1. An explanation of certain criteria which must be met in order to complete the mortgage process;
        2. A realistic estimate of the number of days we require to make a loan decision based on normal
           circumstances
        3. The title and address of the person to whom to direct questions or comments.

GENERAL INFORMATION, INTEREST RATE OPTIONS & FEES:

Depending on the type of loan program for which you apply, various fees are charged and certain criteria must
be met. Your loan will be processed and a decision will be made by us according to the guidelines applicable to
the loan program you have selected.

APPLICATION AND PROCESSING

Our application fee is $495.00 and is not refundable except as outlined in NJAC 3:1-16(e).
We estimate that the number of days necessary to make a decision on your application is 14 days from the date
of our receipt of a formal, completed application. For timing purposes, the start date shall be the calendar date
following our receipt of a completed application which includes a check for the application fee. If you withdraw
your application after the appraisal is competed, you are responsible for payment to appraiser.

                 Summary & Explanation of Typical Closing Costs
Because we work with many lenders to obtain the best rates and programs for you, some
costs and funds needed at closing can vary. Once you receive your good faith estimate,
the fees listed on it will be much closer to the final amounts. This summary is intended to
help you understand those costs & fees.

Application fee – $495 for a residential application, and $595 for an investment property. It
pays for all of the processing and the coordination between you, the bank, your attorney or
Title Company and us. NO FEE ON FHA LOANS UPFRONT, EXCEPT APPRAISAL COD.
Origination Fee – There are typically no origination fees or points unless we discuss them
in advance;
Property appraisal – Cost is about $300-750, 400.00 for FHA LOANS. Some properties
valued above $500,000 or a Multi-Family may cost more to appraise.
Bank commitment fee - This is the fee banks charge to have their attorney review the file.
The banks I use most often charge $300 - $850;
Credit Reporting Fee - $18-24 for all 3 credit bureaus’ reports on your credit status;
Title work – WE WILL ORDER TITLE ON REFINANCES. On a purchase, most people use an
attorney who arranges all the title and deed work. For refinancing, most clients just use a
title company. Title work is regulated by the state and the cost for a purchase in NJ is $5.25
per thousand up to $100,000, and $4.00 per thousand for every thousand above that.
Refinances are slightly less. If the title company closes the loan for you, they will charge a
closing fee of about $300 - otherwise, your attorney will be paid for that (usually about
$500 - $800);
Govt. & recording fees - Deed - $90, tax service fee $75, flood certification $20,and a $28
filing fee;
Wire transfer fees – Some banks and title companies will charge a fee for this (about $25 -
$50);

Other funds needed for closing are 3 months of taxes to be put in escrow, proof that your
homeowner's insurance is paid for the first year plus an additional 3 months worth of
homeowner's insurance payments put into escrow. If you have taxes in escrow with your
current bank, they will refund those to you within 30 days – but keep in mind that you’ll
need the 3 month’s tax escrow money for the new loan;
Interest - Since interest on your mortgage payments are paid in arrears, if your closing
were on June 15th, your first mortgage payment would be on August 1st (interest for July
will be paid with the August payment). At the closing, though, you would need to pay the
15 days of interest on the loan amount for the 15 remaining days in June. On a loan of
$200,000 at 6.0%, you will be charged $499.95 for those 15 days in June ($200,000 X 6% =
$12,000 ÷ 360 days = $33.33 per day). $33.33 X 15 days = $499.95.

Once you make application for your loan, you will receive a good faith estimate (GFE) with the
costs and fees of your loan on it. Also, please note that in some cases and for varying reasons,
the actual bank we work with may change during the process (better rate, appraisal issues, etc). I
