How to request face to face Sign Language or CART/RTC interpreters:

   1. Choose an agency from the above list that provides face to face contact or CART/RTC
      interpreters.
   2. Fill out the Interpreting Request Form (do not fill out below “acknowledgement of request”
      - this is for agency to fill out only).
   3. Fax or Email form to the appropriate agency (Information listed above).
   4. The agency will contact you by phone, fax, or email to confirm the availability of an
      interpreter (if agency cannot fill the request, you will need to select another agency from
      the list above and repeat steps 1-3).
   5. When services are rendered, a Verification Form for interpreting services will need to be
      signed. You will need to identify who in your local office is able to sign this form and who
      should receive the local office copy (The interpreter will submit their copy with the bill).
   6. Keep in mind that if requesting an interpreter 4 days in advance the rate will be less than
      requesting interpreter less than 24 hours (rates are listed in https://procure.az.gov).

How to request VRI (Video Remote Interpreting)

   1. Choose an agency from the above list that provides VRI Services.
   2. Fill out the Interpreting Request Form (do not fill out below “acknowledgement of request”
      This is for agency to fill out only).
   3. Fax or Email form to the appropriate agency (information listed above)
   4. The agency will contact you by phone or email to confirm the availability of VRI Services
      (if agency cannot fill the request, you will need to select another agency from the list
      above and repeat steps 1-3.
   5. The agency will contact you to confirm availability of an interpreter and set up VRI
      Services (at this time the IP numbers will be exchange).
   6. Keep in mind that if requesting an interpreter 4 days in advance the rate will be less than
      requesting interpreter less than 24 hours. (rates are listed in https://procure.az.gov).
   7. Note: Because this service uses Video Phone Technologies and the interpreter does not
      need to travel, interpreting may be readily available within the same day (Note: there
      may not be a verification form to sign).
