Payment Request Form Training Text Slide Title Narration 1 Introduction Welcome to the Ticket to Work Training Series. In this training session, we will discuss the Payment Request form. 2 Navigation Let’s begin by discussing how this presentation works. Click on the document name to begin the download. 3 Vision of Associate Let us take a moment to introduce the vision of Bob Williams, the Commissioner for Associate Commissioner of the Office of Employment Support Programs OESP with Social Security. In order to improve upon the availability and quality of employment support programs, Mr. Williams has laid out his guiding principles for the Ticket to Work Program, which he calls the Five Cs. First, we will increase the availability and quality of choices available to Ticket Holders. By providing more choices to Ticket Holders, they are better able to obtain the skills they need and select employment that will lead to self-sufficiency. Second, we want to increase the capacity of the service providers. The idea behind this is to increase both the number and quality of the service providers to ensure employment and self-sufficiency outcomes for beneficiaries. Third, we will require greater coordination and collaboration among our employment support providers. This ensures that services are provided in the most simple, easy to understand, and straightforward manner as possible. Finally, we will also ensure that all of our employment support programs produce increasingly credible results. There are many paths to success, so we need to make sure that we open those paths that can improve the performance of the Ticket to Work Program. Throughout this session, you will learn how the Payment Request form is used to ensure Mr. Williams’ final guiding principle: ensuring our employment programs produce increasingly credible results. 4 Course Objectives Upon completion of this training course, you will be able to: Determine when a payment is available for your Employment Network; Differentiate between evidentiary payment requests and certification payment requests; Complete the Payment Request form; And identify resources to aid you with the Payment Request form. 5 When to Request a As an Employment Network, you are entitled to receive payments for the Payment services you provide that help beneficiaries along the path to self- sufficiency. In order to receive these payments, you will need to complete and submit the Payment Request Form. This starts by determining whether a payment is available. If you had assigned the Ticket under the Milestone/Outcome payment method, then you may be eligible to receive a payment once the Ticket Holder has earned the Trial Work Level amount. Phase I Milestone 1 can be paid once the Ticket Holder has earned the Trial Work Level amount for one month. This payment may also be available if the beneficiary did not work the entire month, but has earnings that would equal Trial Work Level if that person had worked the entire month. Phase I Milestone 2 can be paid once the Ticket Holder has earned the Trial Work Level amount for three months within a six month period. Phase I Milestone 3 can be paid once the Ticket Holder has earned the Trial Work Level amount for six months within a 12 month period. And Phase I Milestone 4 can be paid once the Ticket Holder has earned the Trial Work Level amount for nine months within an 18 month period. Phase II Milestones are paid when the Ticket Holder reaches Substantial Gainful Activity level earnings but still receives benefits from Social Security. All Phase II Milestone payments are based on SGA earnings and single calendar months. Unlike Phase I Milestones, each month is treated individually. Outcome payments are made when the Ticket Holder reaches Substantial Gainful Activity and has stopped receiving benefits. In order to receive any of these payments, the Payment Request form must be completed and submitted. 6 Payment Request Before we take a look at the form, we need to discuss the two types of Types payment requests: evidentiary and certification. 7 Evidentiary Request Let’s begin with evidentiary requests. These requests are based on primary evidence showing that the beneficiary achieved the required level of earnings for the EN to receive a payment. Primary evidence includes pay stubs, earnings statements prepared and signed by the employer, or earnings statements from The Work Number, which is a third-party service that provides monthly earnings amounts. Whatever the primary evidence, it must include: The beginning and ending dates of the pay period, along with the actual pay date; Gross earnings for the claimed month; The year-to-date gross earnings; And withholdings. The only exception is that if The Work Number is used, then withholdings do not need to be present. 8 Certification Now let’s take a look at the second type of request, which is a Request Payment certification payment request. This request option makes it possible for Option your EN to receive a payment even when you are unable to obtain primary evidence, such as a paystub. In this case, the first thing you will need to do is include secondary earnings information. There are several types of secondary information. One type is a recent dated contact. This is where you have had recent contact with either the beneficiary or the employer and confirmed that the beneficiary has earnings that qualify for either a milestone or outcome payment. If you are using a recent dated contact, you will need to include the type of contact that occurred, such as a phone call or an email, the date when the contact took place, and a description of the information you learned regarding the beneficiary’s earnings level. The second is an Earnings Inquiry Request response you had received from MAXIMUS. This includes Earnings Alerts you receive if you are signed up for Universal Auto-Pay. The third is data you had received from the National Directory of New Hires, which hosts beneficiary earnings information. And the fourth is the Self-Employment Income Form, which would only apply if the Ticket Holder is self-employed, and a Milestone payment is being requested. The actual Self-Employment Income form must be completed and signed by the Ticket Holder, and must include monthly gross income, monthly gross expenses, and a signed statement from the beneficiary certifying self-employment. The second item you’ll need to include as part of a certification request is a repayment agreement. This agreement states that the EN will either repay the amount or allow the amount to be deducted from future payments if it is determined later that the EN was not entitled to the payment. It should be noted that certification requests take longer to process. This is because secondary information is considered “alleged” earnings or earnings leads. As a result, Social Security will need to verify these earnings before any payment can be made. 9 Check Your Let’s take a moment to check your understanding of the material we’ve Understanding covered so far. This is strictly for your understanding; no score is given or collected. 10 Check Your Understanding 11 Payment Request Let’s now move into the actual form, which can be downloaded from the Form www.yourtickettowork.com website. In the Employment Networks section, click on Forms. The Payment Request form is located under the Payments header. 12 Employment Section I of the form asks your EN’s name and DUNS number. Next, you’ll Network want to state whether your EN’s bank account information is current with Information the Central Contractor Registration. If it is not, you will need to visit www.ccr.gov to update the information. Any incorrect or outdated information will prevent payment to the EN. 13 Ticket-holder Section II asks for information about the Ticket Holder. This includes the Information Ticket Holder’s name, Social Security Number, employer, and the employer’s address, if available. The final item in this part is the payment method. Mark whether this Ticket was assigned under the Outcome-Only payment method, or the Milestone-Outcome payment method. 14 Phase 1 – Milestone Section III of the form needs to be completed when requesting a Phase I 1 Earnings Milestone 1 payment. If you are requesting a Phase I Milestone 1 Information payment, then you would need to indicate whether the beneficiary earned Trial Work Level during the claim month; earned less than Trial Work Level, but expects to earn that level within the next two months; or earned less than Trial Work Level and does not expect to earn that level within the next two months. Please note that it is extremely important that you complete this section if you are requesting a Phase I Milestone 1 payment. If this section has not been completed, the payment request will be denied. 15 Payment Request In Section IV, indicate if this is an Evidentiary Payment Request or Details Certification Payment Request. Also, you will need to provide the claim month and year for this payment request. If there are multiple months, list all months for which the payment request applies. 16 EN Services Details Section V of the Payment Request form needs to be completed when requesting payments for Phase I Milestones 1, 2, and 3. If you are requesting a Phase I Milestone 1 payment, you will need to list the services that were provided to the beneficiary as well as the dates that those services were provided. For Phase I Milestones 2 and 3, you will need to detail those services that were provided since the last milestone payment month. If this information is not included with the payment request, the Milestone payment will be denied. There is a note on the bottom of this section stating that, in addition to the Payment Request form, the Certification of Services must be submitted along with certain payment requests. For more information about the Certification of Services, you may review the training available on www.yourtickettowork.com. 17 Evidentiary Earnings If you are making an evidentiary payment request, you will need to Information complete Section VI of this form. In this section, you will need to indicate the type of evidence you are submitting along with the request. As mentioned previously, this may be a pay stub, an earnings statement prepared and signed by the employer, or information from a third party source. 18 Certification If you are making a certification payment request, then you need to Payment Request complete Section VII of the form. First, you will need to indicate the type Details of certification information that is being submitted along with the request. As mentioned earlier in this session, this may be recent contact with the beneficiary or the employer, an Earnings Inquiry Request response or Earnings Alert, data from the National Directory of New Hires, or the Self Employment Income form. If you select one of the first two options, recent contact with the beneficiary or employer, then you need to provide details of that contact. This includes the type of contact, such as email or phone call, the date of the contact, and a description of what you learned regarding the beneficiary’s earnings. 19 Repayment The final two sections of the form are shown here. Section VIII is the Agreement and Repayment Agreement, which is mandatory for certification requests. Contact Information This agreement states that the EN will either repay the amount or allow the amount to be deducted from future payments if it is determined later that the EN was not entitled to the payment. You will need to sign this section and provide the date of the signature. Section IX simply asks for the contact information of the EN representative who is requesting the payment. In this section, you’ll need to provide your name, phone number, fax number, and email address. 20 Submitting the Form Once the form is complete, you may fax the form, and any evidentiary or certification documentation, to MAXIMUS at 703-893-4149. You may also mail the request to MAXIMUS using the address given on this slide. 21 Processing the Form After MAXIMUS receives your payment request, the documents will be reviewed to ensure the necessary information has been included. Additionally, MAXIMUS will evaluate the documents you submitted to verify authenticity, and certify that what was submitted is sufficient evidence to document earnings. If the information is incomplete, an Account Manager will contact you to acquire the missing information. When the payment request is processed and approved, Social Security will electronically transfer the payment to the established EN bank account on file. In some cases, the payment request is denied. If this occurs, the EN will be notified of the denial via the EN Payment Status Report. 22 EN Payment Status Let’s take a look at that report. Every Friday, your Employment Network Report will receive weekly status reports of all payment requests received by MAXIMUS. As shown here, the report reflects year-to-date payment activity data and includes: The date each payment request was received by MAXIMUS, Beneficiary name, Payment option, Dollar amount paid, Claim months, And the status of each payment request submitted. This report is the best way to determine whether a payment has been denied or approved. If 30 days have passed since your payment request was sent to Social Security by MAXIMUS, and a payment or denial has not been issued, you may contact your Account Manager for more information. It is possible that the case was put on hold for additional processing by the Social Security field office, or that there is additional information that must be submitted. 23 Check Your Let’s take a moment to check your understanding of the material we’ve Understanding covered so far. First, you can call the toll free helpline for assistance from the Technical Assistance and Support Center, or TASC. TASC was established to address technical assistance needs of the participating ENs. In doing so, TASC can provide regional and functional services to help enhance internal procedures, increase Ticket assignments and payments, and ensure more Ticket Holders access and retain employment. Additionally, service providers can visit www.yourtickettowork.com to obtain additional information and review available training materials. Finally, once a month, both MAXIMUS and Social Security host a call to discuss changes and emerging issues regarding the Ticket to Work Program. There are two separate calls: one for ENs, and one for VRs. The All EN Call takes place on the first Thursday of every month, while the All VR Call takes place on the second Tuesday of every month. Please contact your Account Manager for more information regarding these calls. 26 Key Points Let’s take a moment to review the key points that were covered in this lesson. First, we discussed how to determine when a payment is available. For Phase I Milestones, the beneficiary must have reached Trial Work Level amounts. For Phase II Milestones and Outcome payments, the beneficiary must have reached Substantial Gainful Activity earnings levels. We then differentiated between evidentiary payment requests and certification payment requests. Evidentiary requests are based on primary evidence showing that the beneficiary achieved the required level of earnings for the EN to receive a payment, while certification requests are based on secondary evidence and require a signed repayment agreement. Next, we discussed completing the Payment Request form. You will need to provide information regarding your EN and the Ticket Holder, and then indicate the type of request that is being made: evidentiary or certification. For Phase I Milestones 1 through 3, you will need to complete the EN Services Details section of the form to include the services that were provided to the beneficiary. You will then need to indicate the types of earnings documentation you are submitting and sign the repayment agreement. To complete the form, you’ll need to provide the contact information of the person submitting the request. Finally, we identified resources to aid you with the Payment Request form. These resources included the TASC helpline, the www.yourtickettowork.com website, and the monthly calls hosted by Social Security and MAXIMUS. 27 This Completes The This completes the Payment Request Form training session.
