Most owners will have the opportunity to work their dog in Saturday’s session. Dogs will also be
needed for Sam’s demonstration on Sunday, but those will be limited to 2 or 3 dogs per behavior,
pre-selected based on the information we receive in this section of the registration forms.
Please arrive before 8:45 am. Each person will have their own area for their belongings (please
bring a chair) and their crate. We do not have overnight accommodations; dogs must leave with
their owners at the end of the day. You will need to bring proof of rabies and DHLPP, a water
dish and a crate.

                                 Waiver of Liability
I hereby acknowledge and agree to this waiver of liability, which is in effect for all
seminar participants. I shall assume all responsibility for the care of my dog through the
duration of the seminar, sanitization of the crates, potty breaks, cleaning up after my dog,
supervising my dog when uncrated to protect my dog, other dogs and people and will not
leave the premises without taking my dog with me.

Michigan Samoyed Rescue, Sam Malatesta, Puppy for Life, and Alpine
Academy are not liable or responsible for any damages, loss, injury or illness to any
person, animal or property stemming from or related to attendance or participation in this
seminar. In registering for and attending this seminar all participants, volunteers and
