                          Refinance Application Instructions
RE: Apt. # _________________

The mortgage refinance application and the financial statement are required. All of the
information is held confidential and is used only for the purpose of the Admissions Committee
and Board of Directors.

In order to expedite processing of the refinance package, the Admissions Committee reserves
their right to hire the Cooperative Accountant to review your financial information at a fee of
$175/hour payable by the applicant(s). Please assemble the following required information and
checks (MONEY ORDER OR BANK CHECKS ONLY) and forward it to the Property Office:

- House Rules Acknowledgment
- Copies of the last 2 years’ tax returns with ORIGINAL SIGNATURE(S)
- Copy of Renters Insurance Declaration
- 2 Weeks’ Paystubs
-$375 Application Processing Fee non-refundable.
-$125 per person for running credit/criminal verification
-Mortgage Refinance Pre-Approval showing monthly payments

The Board of Directors’ approval and inspection of the apartment must be obtained before
closing. As soon as all of your information is received, a date will be set for you to meet with the
Admissions Committee and final approval by the Board of Directors. If the application is
approved, an appointment to inspect the apartment will be made.

Please be advised that your bank will prepare a Recognition Agreement which must be reviewed
by the Kings Wood attorney. After our attorney’s review and approval, a Board member must
sign the Recognition Agreement. The Recognition Agreement is returned to the Kings Wood
attorney and then presented to your bank for closing. Note that this can take several weeks.
Additionally, please note that our attorney’s closing fee for $300 must be paid at closing.

This will confirm that I have read and understand the Kings Wood Owners’ Corp. House Rules and Regulations.

As written in section 4.18 of the House Rules, it is required that new residents must comply with the following
specifications.

4.18 Flooring Specifications:

      All Shareholders owning prior to May 1, 2005:
      The floors of each Apartment must be covered with rugs or carpeting or equally effective noise-
      reducing material, to the extent of at least eighty (80%) percent of the floor area of each room
      excepting only kitchens, bathrooms, closets and foyer.

      Current Residents:
      Effective May 1, 2005, all current residents remodeling their floors must use carpeting or
      Pergo type flooring with rubber backing. Each apartment must be wall to wall carpeted
      throughout excluding kitchens, bathrooms, closets, 1st floor dinettes and 1st floor foyers. If a
      wood floor look is desired, a laminated floor typical of a Pergo floor with rubber backing can
      be substituted for carpeting and must be used in conjunction with area rugs and padding to cover
      at least 80% of the floor (see padding specifications below). No hardwood sanded floors are
      acceptable anywhere on the second floors. A copy of the invoice of material purchased and
      installed must be submitted to the Kings Wood office.

      New Shareholders:
      Effective May 1, 2005, all new shareholders must use carpeting or Pergo type flooring with rubber backing.
      Each apartment must be wall to wall carpeted throughout excluding kitchens, bathrooms, closets, 1 st floor
      dinettes and 1st floor foyers. Hardwood sanded floors are NOT acceptable anywhere on the 2nd floors.
      If a wood floor look is desired, a laminated floor typical of a Pergo floor with rubber backing can be
      substituted for carpeting and must be used in conjunction with area rugs and padding to cover at least 80% of
      the floor (see padding specifications below). Hardward floors are permited on 1st floor with area rugs.
      The Shareholder must be in compliance with these Flooring Specifications within 90 days from closing. A
      copy of the invoice of material purchased and installed must be submitted to the Kings Wood office.

      Padding Specifications: “Rebond” foam 6 to 8 lbs padding, ½” thick, with thermo bond paper under the
      padding which acts as a sound muffler. (This is an effort to reduce noise pollution between apartments.)

                          Requirements for Contractors Working on
                            Kings Wood Owners’ Corp. Property
If you hire an outside contractor, you are responsible for advising them of the following rules:

     1) Parking - Contractors who park in reserved parking spots in order to unload
        materials or remove debris may park in a reserved parking spot only for a
        limited time and must remove their vehicle upon request of the shareholder
        whose reserved spot they are parked in.

          After unloading material and/or tools, the vehicle must park in a Visitors
          parking spot unless authorized by the Property Manager.

     2) Debris - Shareholder is responsible for removal of appliances, carpet,
        furniture, etc. from the property and shall remove all of the above and debris
        at his/her expense immediately. Contractors must remove all debris from
        the worksite. Small debris may be placed in the dumpsters, not alongside
        the dumpsters.

         A full clean-up of the common hallway must be done at the end of each day
         with a vacuum and/or mop as needed.

     3) Trash - Contractors must remove all trash including soda cans, cups, food
        containers, etc. Throwing of cigarette butts onto walkways, parking lot or
        lawn is not permitted.

     4) House Rules - All work performed by contractors must be in accordance
        with the Kings Wood House Rules and approved by the Property Manager.
        All work may only be performed between the hours of 8:00 a.m. to 6:00
        p.m. on weekdays and between the hours of 10:00 a.m. to 6:00 p.m. on
        weekends.

All shareholders and residents must inform contractors they hire of these requirements before
performing work on the Kings Wood property.
