TO RECEIVE YOUR BLINDS.COM MAIL-IN REBATE:
1. Purchase a qualifying product(s) during a valid mail-in rebate offer period (May 10, 2007 - July 31, 2007). Rebate
amount will be determined below. Your packing slip is in the clear, plastic envelope affixed to the outside of the box of
your new blinds. Your purchase Order Confirmation is electronically sent to you at the time of your order. Keep both of
these to turn in with your rebate.
2. Complete this form. You must sign the agreement below.
3. Mail this completed form, along with your packing slip, purchase Order Confirmation, and hanger coupon, to the
address listed below. Your request must be postmarked within 30 calendar days of your purchase date (“date” as listed
on your purchase Order Confirmation). Please allow 6-8 weeks for processing.

Blinds.com Mail-In Rebate offer valid only with eligible U.S. purchases during valid mail-in rebate offer period (May 10, 2007 – July 31, 2007). Offer good
while supplies last and no substitutions or rainchecks - available to new customers only. Free blind rebate is defined as a maximum value of $50. Any
purchase under $50 will be rebated for the exact value of the blind, up to $50, and any purchase over $50 will be rebated for $50. Offer may be combined
with other site discounts available from Blinds.com, but cannot be combined with any other coupon offers. The coupon code MAFREE must be entered
during checkout to qualify order for rebate. By entering code MAFREE, the customer agrees to the terms & conditions listed on this page. Note that the
$4.95 order processing will not be rebated - customer is responsible for this cost, as well as any applicable sales tax (Texas only). Request form must be
fully completed and must include a copy of dated packing slip and purchase. Order Confirmation for purchase made direct from Blinds.com for qualifying
product purchased. Keep copies of all materials sent. Materials received become the property of Blind.com and will not be returned. REBATE IS FOR NEW
CUSTOMERS ONLY, ONE REBATE PER QUALIFYING PURCHASE/PER PERSON/PER HOUSEHOLD/PER BUSINESS. Blinds.com will extensively
with the terms and conditions, and will not issue rebates when terms and conditions are not met. Blinds.com is not responsible for lost, late, or misdirected
mail. Request must be postmarked within 30 calendar days of purchase date. Any claims postmarked after 30 calendar days of purchase date will not be
processed. If terms and conditions are not met, rebate will not be sent. Please allow 6-8 weeks for receipt of check. Rebate checks are void if not cashed
within 90 days of issuance and cannot be reissued. To qualify for this rebate you must be a legal U.S. resident 18 years of age or older. The description of
Blinds.com mail-in rebate offer must appear our Web site at the time of your purchase. Only actual purchaser of the qualifying products may participate in
this rebate program. Requests from group or organizations will not be honored. Void where prohibited or restricted by law. Availability and terms of offer
may change without notice. Please note that this offer is for first-time customers only, and Blinds.com reserves the right to refuse rebate payment to anyone
