AUSTIN O’BRIEN HIGH SCHOOL COMMITMENT FORM FOR 2011 FOOTBALL TRIP TO CALIFORNIA Practice Itinerary from August 18th – August 23rd 2011. These practices and spring practices are mandatory if you are to participate in this trip! Thursday August 18th and Friday August 19th practices 6:30pm-8: 30pm. Saturday Aug 20th and Sunday Aug 21st practices are two a day’s from10:00am-12:00noon and from 2:00-4:00pm Monday August 22nd and Tuesday August 23rd practice from 6:30-8:30pm. DEPART FOR SAN FRANCISCO ON WEST JET WED AUGUST 24TH 8:30AM. A detailed itinerary will follow based on the NFL preseason schedule released in mid April. However, Wednesday we will arrive mid afternoon and go to Sacred Heart prep to drop off our equipment then check in at the Best Western Riviera 15 El Camino Real. Practice after check in, supper then relax at the pool. Thursday will include practice, and a tour of Stanford athletic facilities as well as the possibility of attending a Stanford practice. Friday morning light walk through practice then back to hotel. Friday afternoon activity TBA, then return to Sacred Heart to to dress for Jamboree at Half Moon Bay High School. Friday Night Lights Jamboree vs Sacred Heart Prep, Half Moon Bay, and Hillside at 7:00pm. Saturday Alcatraz tour and NFL preseason game in Oakland. Sunday depart Best Western by bus to Anaheim, we will stop at the outlet stores in Gilroy then arrive at Carousel Inn and Suites 1530 S. Habor Boulevard late Sunday. Monday and Tuesday we will be at Disneyland. DEPART FOR EDMONTON ON WEST JET WED AUG 31ST 12:05 ARRIVE 4:14PM All additional student information sheets, the International Parent Permission Form, identification forms, custom forms etc. will be handed out at a later date. Parents meeting with the AOB staff will also take place when all final details have been arranged in late April. COST PER PLAYER IS $1700.00 based on 36 players traveling and includes the following: Air & ground travel, hotel accommodations, taxes, deluxe travel insurance, 2-day Disneyland park pass, NFL game / Alcatraz Tour, and Coach’s flight and accommodations (based on 8 coaches traveling). The $1700.00 does not include meals or spending money. Please note that the final cost is subject to change depending on the number of players and coaches traveling at the time of booking. PAYMENT SCHEDULE The first installment of payment in the amount of a $500.00 deposit by [credit card, debit, cash or cheque made payable to Austin O’Brien High School] is due on or before Wednesday March 23rd.You can phone in payment at the business office if more convenient. If by Wednesday March 9th we do not have enough students to a field team with enough players in each position your deposit will be returned to you. A second installment of $600.00 is due on or before Tuesday May 17th 2011. The balance of payment 600.00 is due Friday June 10th 2011. The school reserves the right to disallow any student from attending the trip if an Austin O’Brien staff member (in consultation with the student’s current Junior High administration if in grade 9) feels this student is not working or performing to school and football program standards. Selection is based upon merit, including performance ability, commitment and overall behavior in all classes and activities. Performance ability. The player must be able to perform at a skill level that does not put himself or his team-mates at risk physically. The player must have put in the required off season weight room training to prepare himself physically as the teams we are playing have mandatory 5 day a week weight room sessions. He must also be able to demonstrate an understanding of offensive and defensive assignments Commitment. Attendance at off-season speed and weight training sessions is required a minimum of 3 times per week even during midget season. Attendance at spring/fall practices is MANDATORY. Behavior. As outlined in the student handbook. FUNDRAISING In 2008 only one family ended up helping me with the Sobeys gift card fundraiser so I would rather not fundraise again. However once we know the trip is a go, I can arrange to have the school for a parent’s organizational meeting if necessary. At the bottom of this form you can indicate if you are interested in fundraising. HAVE THE PLAYERS TAKE SOME RESPONSIBILITY By only working one 8 hour shift a week the boys can easily help supplement the cost of the trip if not pay for it all. PASSPORTS Everyone must have a valid passport to travel to the USA by air. The way to avoid the three-hour line-ups is to do your application on line. After the pre screening line you will only wait minutes to see an agent instead of hours.
