					Operational Procedures Manual


   AFC Management Inc.
Shift Duties:

    1. Arrive to all shifts on time and at appropriate gym and in appropriate attire.
            a. Alpha Fitness shirt or Polo to be worn with tan/black pants/shorts.
    2. Collect dirty towels from towel bins and wash and dry them promptly.
    3. Fold and store towels in cabinet.
    4. Wipe down locker room counters, mirrors, etc. in between the morning and lunch crowds and
        the lunch and after-work crowds.
    5. Replace towels that have been used.
            a. Should be at least 8 shower towels out and 8 sweat towels.
    6. Wipe down all equipment before shift is over.
    7. Record every member that comes in and which key they are given.
    8. Return keys promptly to hook and record that they have been returned.
    9. Complete any tasks assigned in personal ‘To-Do’ Folder (Located at each gym with staff name)
            a. Completed tasks are to be placed in Manager Inbox
    10. Greet every member with a smile.
    11. Any other duties as may be required.

Opening Duties

    1.   Arrive at least 15 minutes before shift starts.
    2.   Turn on all lights.
    3.   Turn on fans.
    4.   Turn on all equipment and TVs.
             a. TV’s should be tuned to Fox News (Ch. 205 in Granite Park 2).
    5.   Put out towels if needed.
    6.   Check washer and dryer for towels to be moved or folded.
    7.   Check supplies in locker rooms and replace any supplies that are less than 1/4 full.
             a. Record date on bottom of new supplies and record the replacement of the product in
                 the Inventory Log.
    8.   Any other duties as may be required.

Closing Duties:

    1.   Clean all equipment.
    2.   Turn off all equipment and TV’s.
    3.   Put any dirty towels in washer.
    4.   Put out towels for next morning.
    5.   Check locker rooms for any miscellaneous items.
    6.   Make sure all keys are accounted for.
    7.   Turn off lights and fans.
    8.   Lock wash room door and front door.
    9.   Any other duties as may be required.
New Member Waivers/Agreements

      New Members are required to register online before using facilities. Access to registration is
       available on Mozilla Firefox at www.allplayers.com/g/alphafitness_staff (DO NOT Log In)
           o Member agreements will be available for any new member that would like a copy. They
                will be located in the File box under Membership Agreements.
      Payments will be taken and placed in lock box in drawer and recorded in payment log
      Fast Fitness/Guest Pass form must be filled out by any customer wishing to use facilities under
       that program
           o Payments will be recorded in payment log
           o Guest pass will be filed in the Trial Members folder.
      Customers wishing to use a free trial week must fill out and sign a waiver
           o Record the member on the trial week sign-up with start date and end date
           o File the waiver in the Trial Members folder.

Invoices

      On the first of every month a collections sheet will be printed, along with invoices and filed in
       the Collections folder.
      When a customer pays the invoice:
            o The payment will be recorded in payment log and on the invoice and collections sheet.
            o The payment will be placed in lock box in drawer.
      Member’s will be given the option to receive invoices by:
            o Email, Mail, Personally, Phone Call
            o Members may choose multiple options and will be contacted in such way as directed
      Members will be notified 45 days prior to the due date, 30 days prior to the due date, 15 days
       prior, 7 days prior and the day of the due date until renewal or cancellation is confirmed or
       payment is received. If no contact is made during this time and no payment is received, member
       will be notified 2 weeks after due date and every 7 days thereafter until the 30th day or until
       payment is received. If no payment is received then the membership will be cancelled and
       member must pay $25.00 reinstatement fee upon re-joining the center.
      If a member gives notice of cancellation, a cancellation report is to be filled out by staff and filed
       appropriately.



Cancellations

      Any member wishing to cancel a membership must give 30 days prior notice before cancellation
       (this waives reinstatement fee if re-joining in future).

Inventory

       Staff Members will be required to take inventory and log inventory at the end of each month.
       When an item is placed into use the staff member will write the date on the bottom of the item
        with a sharpie. And record the replaced item in the Inventory Log.
       These logs will be checked bi-weekly to insure back stock is sufficient and back stock will be
        purchased quarterly.
