(Cheques must be crossed and made payable to the “Singapore Dragon Boat Association”)


   1.      Entries must be accompanied by entry fees and submitted by hand or by post to the
           Singapore Dragon Boat Association.


   2.      Entry fees are not refundable.


   3.      This form must be duly completed and printed in capital letters.


   4.      This form must reach the Singapore Dragon Boat Association, Kallang Water Sports
                                                                 th
           Centre, No. 10 Stadium Lane, Kallang by 12.00 pm on 30 May 2008 (Friday).


   5.      Upon submission of this confirmation of participation we confirm that we have read and
           understood the Entry Rules and the Racing Rules and Regulations and agree to abide
           by them. We note that the Singapore Dragon Boat Association and its co-organizers
           reserved the final right in determining the event that my Company/Organization will
           participate in.
