Trail Design Review Form
Purpose: The Department of Forests, Parks & Recreation requires all trails funded under the
Vermont Recreational Trails Grant Program be reviewed and approved by a professional with expertise in trail design. This process helps assure
trail safety and integrity.

Process: Review the reference list of Trail Design Professionals on the reverse of this form. The list is not comprehensive and serves only as a
concise reference list. Provide the individual/organization with this form, topographic maps, diagrams of trail components and any other
necessary information. On-site assessments are recommended, but not required. This form must be completed and signed by a Trail Design
Professional before any reimbursements will be processed. Submit and retain a copy of this form.

Note:    The service of a Trail Design Professional is an eligible project cost. Please review procurement procedures in the Application
Supplement for guidance.
