I am very pleased to offer you the position of (position). The terms of the offer are as follows. [Note: where immigration documents such as an
Employment Authorization are required, the offer should be made conditional upon the candidate obtaining these documents (Section 2.6 – Conditional offers).

CLASSIFICATION & STATUS (Sections 1.2, 2.8 & 2.9):

SALARY (Section 4):

1. The salary offered is ($ per month). Continued employment is dependent upon satisfactory performance and the availability of funding.
OR
2. The salary offered is ($ per annum or $ per month). Efforts will be made to review your performance and salary on an annual basis. Salary
   adjustments and the continuation of this agreement are dependent upon satisfactory performance and the availability of funding.


BENEFITS (Sections 5 & 6):

1.   Limited term position of six (6) months or more with Fringe Benefits

     The mandatory benefits that you are covered by include Employment Insurance [EI] and Canada Pension Plan [CPP]. Deductions for EI and CPP
     are made in accordance with the terms of these plans.

     You will accumulate (# days - prorate if part-time) vacation for every month of service. Vacation is to be taken after it is earned and your
     entitlement should be used by the end of the year following the year in which it is earned. Annual vacation plans should be arranged with your
     supervisor, (name of supervisor) and records of sick and vacation accruals use should be submitted monthly. Please note that under normal
     circumstances, carry over of unused vacation is not permitted. If exceptional circumstances exist, a written request must be submitted to your
     supervisor) and written permission to carry over unused vacation must be obtained.

     You are also entitled to the following fringe benefits: (list here - i.e. Dental, Personnel only), Basic Group Life Insurance, Voluntary Group Life
     Insurance, Accidental Death & Dismemberment Insurance). You will be required to meet with the resource person in the Human Resources
     Division to complete the required enrollment forms. [Note: Refer to the list of benefits for research individuals, Appendix XIX and XX and
     corresponding documentation. Eligibility for these benefits is based on the type and length of the appointment and the ability of the grant/contract to
     fund these benefits].
OR
2. Limited term position or without term position of two (2) years or more with Fringe Benefits

3.   Limited term or without term position with no Fringe Benefits

     The mandatory benefits that you are covered by include Employment Insurance [EI] and Canada Pension Plan [CPP]. Deductions for EI and CPP
     are made in accordance with the terms of these plans.

     You will accumulate (# days - prorate if part-time) vacation for every month of service. Vacation is to be taken after it is earned and your
     entitlement should be used by the end of the year following the year in which it is earned. Annual vacation plans should be arranged with your
     supervisor, (name of supervisor). Please note that under normal circumstances, carry over of unused vacation is not permitted. If exceptional

     Sick leave credits will be provided at the rate of one and one-quarter days for each full month of service (prorate if part time). Sick leave will be
     cumulative.

     Records of sick and vacation accruals and use must be submitted monthly.

     There are no other Fringe Benefits associated with this position.


DUTIES (Section 2.11):

In summary, your duties will be as follows:


NOTICE (Sections 3 and 2.13):

The hours of this position are: (#) a.m. to (#) p.m. on (list the days of the week). This period includes a (#) minute, unpaid lunch break. You will be
given notice of any change in this schedule.

Notwithstanding the terms of this contract, as referred to in this letter, this contract may be terminated by giving the following “Notice of Termination”:
       i)   One week’s written notice of the period of engagement is less than one year
       ii) Two weeks written notice if the period of engagement is one year or more but less than three years
       iii) Four weeks written notice if the period of engagement is three years or more but less that five years
       iv) Six weeks written notice if the period of engagement is five years or more but less than ten years
       v) Eight weeks written notice if the period of engagement is ten years or more
Notwithstanding the above, the contract can be terminated immediately for just cause.

You will be expected to give as much notice as possible when terminating, but in any event will be expected to give not less than (fourteen days) notice.

OTHER CONDITIONS (Sections 2.5, 8, 2.6 and 2.7):

As a condition of this contract you are required to sign a “Memorandum of Agreement “ regarding intellectual property.

All work shall be conducted in accordance with University safety policies and procedures and statutes pertaining to health, safety and environment.

This position is not in scope of any bargaining unit.

[Note: Where the possession of appropriate legal qualifications, professional designation, or license is required, this should be included in this section.
       Where moving expenses or other allowances are provided for, these should also be included in this section.]

This offer and any agreement arising out of this offer is subject to the condition that it be approved and signed by (the individual or
individuals which you required to approve and sign it, i.e. for appointments of 6 months or more of Professional Research Personnel – the
Vice President (Research) and for Administrative Research Personnel – the Chair of the Appointment Committee ) and in the event that this
condition is not fulfilled or performed, this offer and any agreement arising under it shall be null and void and of no effect whatsoever.
