Christian Preschool and it’s staff permission to apply sunscreen
provided by me to my child, ____________________________ for the
entire 2011 – 2012 school year.

Sunscreen guidelines

   1.   Permission slip needs to be signed for each child every year.
   2.   Bottle needs to be a brand that does not contain medicinal ingredients.
   3.   Bottle needs to have student’s name clearly labeled.
   4.   Bottle needs to be sealed in a Ziploc type bag.
   5.   Bottle will be kept in student’s backpack and not stored at school.
