                                                      Employee will be responsible for the additional (1.43%) of the employee share
                                                      of gross total wages.

Term Life Insurance    WCFD pays premium              Must be contracted for a minimum of 30 hours per week. Fill out application
                                                      when hired. Must be received within 30 days of hire date. Two (2) month
                                                      waiting period. (An employee who completes 10 or more days during the month
                                                      shall be deemed to have completed one month of service). Benefit varies based
                                                      on age.

VOLUNTARY PAYROLL DEDUCTIONS

Wyoming Child & Family Development, Inc. offers employees the opportunity to purchase the below-listed insurance
and/or investment products as optional benefits. By so doing, WCFD does not personally endorse or advocate the
companies involved or any of the products offered. Employee participation is strictly voluntary. Premiums paid for
products marked * may be eligible to submit to the Medical/Dental/Vision Reimbursement Plan. Employees may enroll
in most optional insurances with a voluntary payroll deduction at any time throughout the year.
 VOLUNTARY PAYROLL DEDUCTIONS (PAID FOR BY EMPLOYEE)
  *   Colonial Accident Insurance           Choose from various options.
  *   Colonial Critical Illness Insurance   Pays lump sum upon diagnosis of critical illness.
  *   Colonial Hospital Confinement         Customized options of hospital insurance.
  *   Colonial Cancer Insurance             Offers various levels of coverage for the occurrence of cancer.
  *   AFLAC Cancer Insurance                Offers various levels of coverage for the occurrence of cancer.
  *   AFLAC Dental Insurance                Choose from 3 levels of coverage. Some waiting periods apply.
  *   AFLAC Accident Insurance              Offers various levels of coverage for accidental injury.
  *   AFLAC Hospital Protection             Offers various levels of coverage for hospitalization benefits.
  *   AFLAC Personal Recovery Plus          Offers various levels of coverage for personal sickness.
  *   AFLAC Personal Sickness Indemnity     Offers various levels of coverage for personal sickness.
  *   AFLAC Intensive Care Insurance        Offers various levels of coverage specifically for intensive care.
  *   AFLAC Vision Now                      Offers various levels of coverage for vision benefits.
      AFLAC Long-term Care Insurance        Offers various levels of coverage.
      AFLAC Disability Income Protector     Offers various levels of coverage.
      NCPERS Voluntary Life Insurance       Enroll within 90 days of employment or during annual enrollment period September-
      (American Administrative Group)       November. Policy amount depends on age.
      Colonial Universal Life Insurance     Only one question to answer if you enroll within one month of employment. Later
                                            enrollments require additional questions.
      AFLAC Life Insurance                  Customized for individual needs.
      Tax Sheltered Annuities (TSA’s)       $200 per year minimum contribution requirements. TSA carrier may calculate maximum
                                            withholding. Horace Mann or Ameritas.
      EMC National Life Insurance           Choose additional life insurance on self or family members. No health questions if
                                            enrolled within 30 days of employment. Later enrollments require questions. Must work
                                            at least 30 hours per week.
