This “Agreement” made, effective as of [date], Between: [Name of Party 1] [Address Party 1], And all of its subsidiary and affiliated entities, and all agents of such entities, in their capacity as such or otherwise, except as specifically provided in this Agreement; and: [Name of Party 2] [Address Party 2] And all of its subsidiary and affiliated entities, and all agents of such entities, in their capacity as such or otherwise, except as specifically provided in this Agreement.
In consideration of the terms and covenants of this agreement, and other good and valuable consideration, the receipt of which is acknowledged, the parties agree as follows: TITLE I. MATTERS TO BE SUBMITTED TO ARBITRATION All claims or disputes of any kind between the parties to this agreement arising out of or in connection with any and all contracts between [Party 1] and [Party 2], including but not limited to, claims for wages or other compensation due; claims for breach of any contract or covenant, express or implied; tort claims; claims for discrimination or harassment, any claims for violation of any federal, state or other governmental constitution, statute, ordinance, regulation, or public shall be submitted to arbitration pursuant to the procedures set forth in this agreement.
