                       Fillmore County Public Health (FCPH) is working with our school district to give the seasonal influenza
                       vaccination to youth at school. Youth are eligible for school-located vaccination clinics if they are a
John Smith             student enrolled in school or a child enrolled in a school-located daycare, preschool, or HeadStart program.

                       The 2010 flu vaccine will protect against all three influenza strains that are expected to circulate this year.

                       at a later time. Thanks to grant funding from the American Reinvestment and Recovery Act (ARRA) there
                       will be no cost to you for vaccine or vaccination administration. In addition, no student insurance
Jody Peterson          information will need to be presented. If your student is younger than age eight they may need a second

                       dose of vaccine spaced four weeks after the first dose. Students needing second doses will also be able to
                       receive them at school. If your student misses one of the vaccinations they may need to receive the second
                       vaccination at some other location such as a medical clinic or public health clinic.

                       At this time, we request that you review and complete the attached School Seasonal Flu Vaccination
                       Consent Form and return it to school no later than October 15, 2010. Copies of educational materials
                       and forms can also be found on the school website. The consent form includes options allowing you to
                       either accept or refuse the vaccination for your student. If you refuse, the vaccination will not be given to
                       your student. Sign and date the included consent form to accept vaccination for your student. We will post
                       the date and time on our website when we get that information. We expect that it will be late October or
                       November. We also expect the time to be from 1:00-3:00. The community will be welcome to get their
                       vaccine after 3:00. If you would prefer to be with your student due to their age or health needs, we request
                       that you complete the consent form and bring your student to a community vaccination clinic held by
                       public health. A listing of public vaccination clinics can be found on the Fillmore County website at:

                       parents. Your student’s health care provider also can answer your questions about the influenza virus and
                       will be able to give your student the seasonal influenza vaccine.
