1.   Please completely fill this form item by item (Notebook, CPU, Hard Disk, add-on Memory, PCMCIA card, AC-
     Adapter, etc.) before any RMA# can be issued.
2.   Please fax this completed form to (510) 608-5889 Attn: Customer Service.
3.   After product problems have been confirmed, Customer Service will contact you and issue a RMA number for you.
     Please clearly mark RMA# on outside of package and enclose a copy of this fax form along with the necessary invoices
     to prove the purchases.
4.   All RMA products must be returned within 15 days of receipt of an RMA number otherwise the RMA number will be
     voided and the package will be refused. Returned products must be secured in safe container. Damage or loss of
     goods during shipment is the sole responsibility of the customer.
5.   CHEM USA is not responsible for any lost or damaged goods which are not list in above listing.
6.   Any out of warranty services will be charged $50.00 estimated fee.
7.   All RMA products must be picked up within 3-month after the problem is fixed. CHEM USA is not responsible for
     any machines that have not been picked up after 3 months.
8.   If you have any questions regarding this RMA request form, please contact our Customer Service Department.
9.   CHEM USA Corp. does not accept liability beyond the remedies set forth in the warranty statement or liability for
     incidental or consequential damages, including without limitation any liability for products not being available for use
     or for lost data or software.
